Standing in manbasket, basket suddenly swung to the left and jarred employee's back.

Employee was using hand drill to purge a blocked line with water (overhead). Coal and water was running down on employee. Drill caught and slapped employee on the left check on his face causing laceration. Drill handle became wet and slipped.

Employee walked down into a work area where an excavator was operating. The operator did not see employee, swung bucket around striking EE down onto the ground.
While employee was decending down steps, while carrying water hose, hose caught at corner of railing pulling employee back enough to lose footing on stair treads. He fell onto right elbow on metal stair tread.
While climbing up loader ladder, employee's feet slipped out from under him. He fell down 2 or 3 steps, his right arm caught on railing and the weight of his body pulled his right shoulder out.
While working on flooring in Plant Employee slipped and hyperextended his right knee.
EMPLOYEE WAS DOING ROUTINE BELT INSPECTION, BENT DOWN TO PICK UP A TOOL AND FELT SEVERE BACK PAIN. EMPLOYEE WAS NOT LIFTING OR CARRYING ANY ADDITIONAL WEIGHT AT THE TIME THE ACCIDENT OCCURRED.
