Employee was helping to tear down a set of steps at the mine office when a board came loose and the nail that was in the end of the board punctured his lower leg. He went to the hospital to have it checked and returned to work the next day.

Employee was bent over installing a belt guard and hit his head while raising up causing a sprain to his head and neck. He did not miss any work. This accident was not reported to the company. The company was not aware of this accident until we were contacted by our Workers Compensation provider.

EE was taking a battery terminal off of the Mac 8 mantrip, when the battery blew up and got dirt in his eyes. The EMT flushed his eyes and then took him to the hospital to make sure he didn't get any acid in his eyes. EE returned the next work day.

Employee was taking a steering jack off of the 488 scoop and smashed his right middle finger against the machine. He worked to the end of the shift. He went to the doctor the next day. The doctor told him he fractured the end of his middle finger.

The roof fall was 40' long x 18' wide x 4-7' thick in the laminated slate roof. The fall was in the belt entry on #7 belt at spad #2602. The fall was cleaned up and re-bolted according to the Rehabilitation Plan. The fall was approximately 500' from the active section.
