Coalburg No 2 Mine Coal

Employee was bent over installing a belt guard and hit his head while raising up causing a sprain to his head and neck. He did not miss any work. This accident was not reported to the company. The company was not aware of this accident until we were contacted by our Workers Compensation provider.

The employee strained his lower back while pulling the cable of the #1 Mobile Roof Support out from under some gob.

Employee was operating the roof bolter and a roof strap fell off the canopy and struck his right arm above the wrist causing a contusion.

Employee's knife was open in his pocket. He cut his right hand when he put his hand in his pocket. He had to get 2 stitches but did not miss any work.

Employee was operating the shuttle car and a piece of coal fell from the mine roof and struck his right arm at the triceps muscle causing a laceration.

Employee was swinging out his side of the roof bolter to install his outside roof bolt and a piece of draw rock fell and struck him on the head and shoulder.

EE's were lifting a miner tram idler to load it on the three wheeler at the end of the track. IE felt a sharp pain in his lower back while lifting.

The employee was letting down the cable of a MRS and a rope hanger struck him in the right eye.

Employee had his right hand on the batteries of the 488 scoop when a piece of cap coal fell from the mine roof and struck him on the right thumb.

Employee was bent over picking up a cable and a piece of rock fell from the mine roof and hit him in the back and neck causing a contusion. He went to the hospital to have it checked and is scheduled to return to work on Monday July 16, 2012.

Employee cut his right hand pinky finger on a roof strap plate while operating the roof bolter.

Employee was operating the #1 shuttle car and a rock fell off the corner of the rib and hit his left arm and left side.

Employee stated he twisted around and fell while installing roof bolts and strained his lower back. He was suspected of drug use and was asked by the superintendent to take a drug test. He decided to quit.

Employee strained his lower back while using a ratchet chain hoist to make a belt splice.

EE was hitting a waterline splice with a hammer and a piece of metal chipped off the hammer and went into his right arm. EE went to the hospital to have it checked and returned to work the same day.

Employee was trying to break loose some bolts on a 10SC shuttle car. The breaker bar he was using slipped and hit him in the forehead causing a laceration.

EE slipped on the mine floor while making a fireboss run. He pulled a ligament and received a contusion on his right ankle.

EE was pulling down old ventilation curtain and strained his lower back.

Employee was helping to move the miner cable and strained his lower back.

Discovered a continuation of the roof fall that is in #6 entry, 2 breaks outby spad #2992. The continuation of the roof fall is 6' thick x 20' wide x 15' long. A rehabilitation plan will be submitted for MSHA approval.

Discovered a roof fall in the #6 entry, 2 breaks outby spad #2992. The fall is approximately 3.5' thick x 10' wide x 10' long. A rehabilitation plan will be submitted for MSHA approval.

EE was loading pallets on a track flat car and strained his lower back.

Employee bent over to pick up a piece of belt and strained his back.

Employee strained his lower back while using a bar to lift the track.

EE was operating the roof bolter when a piece of draw rock fell from the mine roof and hit his upper left leg causing a bruise. He returned to work the next day.

Discovered a roof fall in the GL4 panel in the crosscut to the left of spad 2715. Fall is approximately 4' thick x 19' wide x 30' long and was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 8700' from the active section.

Discovered a roof fall in the GL4 panel in the intersection 1 break outby spad 2662. Fall is approximately 4' thick x 20' wide x 20' long and was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 9600' from the active section.

Employee was walking on the section after building a stopping when he slipped and broke his right ankle. He went to the hospital and they put a plate and screws in it.

The employee was jacking up a rail mantrip and a crib block fell on his right hand. He went to the hospital after work and got an X-ray. His right hand had a fracture and they put a brace on it. He returned to work the next day.

Discovered a roof fall in the GL4 panel 1 break left of spad #2580. Fall is 4' thick x 19' wide x 75' long, was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 7200' from the section and has been dangered off.

Discovered a roof fall in the GL4 panel 1 break left of spad #2594. Fall is 7' thick x 19' wide x 40' long and 7' thick x 19' wide x 30' long through the crosscut. This area was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 7200' from the section and has been dangered off.

Discovered a roof fall in the HL1 panel 1 break inby spad #3104. Fall is 4' thick x 20' wide x 20' long in the intersection and was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 2700' from the section and has been dangered off.

Discovered a roof fall in the HL1 panel that fell from spad #3098 thru the break to spad #3102. Fall is 4-5' thick x 20' wide x 70' long in both intersections and the break. This area was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 2700' from the section and has been dangered off.

Discovered a roof fall in the HL1 panel at spad #3117. Fall is 4' thick x 20' wide x 25' long in the intersection and was bolted with #5 x 48" fully-grouted resin roof bolts. This area is 2700' from the section and has been dangered off.

EE was shoveling on #7 head and pulled a muscle in his lower back. He worked to the end of the shift.

EE was taking a battery terminal off of the Mac 8 mantrip, when the battery blew up and got dirt in his eyes. The EMT flushed his eyes and then took him to the hospital to make sure he didn't get any acid in his eyes. EE returned the next work day.

Employee was lifting a piece of belt on #1 tailpiece to put in for skirt rubber when he felt something pop in his back. He reported this accident on 07/27/09 and is scheduled for an MRI on 08/04/09.

Discovered a roof fall in #7 entry, 1 break outby spad #2991. Fall was in the intersection and measured 5-1/2' thick x 20' wide x 30' long. This area was bolted with 4' and 6' fully-grouted resin roof bolts. Fall is 2000' from the active section. The fall has been timbered off.

Discovered a roof fall in the #5 room on the HL1 panel 1 break inby room spad #3115. Fall is 5' thick shale x 19' wide x 19' long and was bolted with #5 x 48" fully-grouted resin roof bolts. This area has been dangered off.

Employee strained his lower back while putting the rub rail back on the miner. His doctor has requested an MRI scan.

Discovered a roof fall in the GL4 panel 3 breaks outby spad 2593 in the #2 entry. Fall is 10' thick x 18' wide x 40' long, 4400' from the active section, developed 9/2007 and bolted with #5 x 48" fully-grouted resin bolts. This area has been dangered off.

Discovered a roof fall in the H mains 1 break inby spad 2972 in #7 entry from the start of the crosscut to the intersection in #6 entry. Fall is 5' thick x 18' wide x 50' long, 1000' from the active section, developed 12/2008, and bolted with #5 x 48" fully-grouted resin bolts. This area has been dangered off.

Discovered a roof fall in the GL5 panel 1 break inby spad #2791 which is 3000' from the active section. It was developed on 6/2008 and bolted with #5 x 48" fully-grouted resin bolts. This area has been dangered off.

Employee was taking a steering jack off of the 488 scoop and smashed his right middle finger against the machine. He worked to the end of the shift. He went to the doctor the next day. The doctor told him he fractured the end of his middle finger.

EE was pulling waterline when he tripped and fell. He caught himself with his left hand and stoved his left shoulder.

There was a roof fall in the rooms, one break right of spad #2778. The fall was 100' long x 15' wide x 10' thick. The area had been bolted with 4' #5 fully grouted resin roof bolts.

Injured was shoveling on #6 belt when he slipped on gob. He fell on a rock and hurt his right knee.

Discovered a roof fall in the rooms on the return side, 3 breaks to the left of spad #2827. The fall was 5' thick x 18' wide x 50' long. This area has been dangered off. The fall is 5 breaks from the active section.

Discovered a roof fall in the GL2 panel near DPP #4 which is 5600' from the active section. It was developed in 9/2005 and bolted with #5 x 48" fully-grouted resin roof bolts. The area has been dangered off.

The employee was loading rock dust from the supply car into the bucket of the unahauler, when he felt something pull in his lower back.

The employee was driving the Mac 8 mantrip and the steering wheel spun out of his hand after hitting a hole. The knob on the steering wheel hit his left wrist and broke his wrist. The employee went to the hospital and they put a cast on his wrist. He returned to work later that day.

The employee was lifting the hydraulic valve chest out of the continuous miner and strained his lower back.

The EE was operating the roof bolting machine and a small piece of rock fell from the mine roof, hitting him in the back of his neck and on his back. He finished bolting and returned to work the next day.

The roof fall was 40' long x 18' wide x 4-7' thick in the laminated slate roof. The fall was in the belt entry on #7 belt at spad #2602. The fall was cleaned up and re-bolted according to the Rehabilitation Plan. The fall was approximately 500' from the active section.

The employee was pulling belt and tripped. He fell on a belt rail bruising his left rib and right knee.

The employment was operating the roof bolter when a piece of rock fell on his left shoulder and left hip. The rock caused a mild contusion on his left shoulder.

The EE was energizing the breaker for the shuttle car at the power center. One of the phases had went to ground in the plug, causing the EE's left hand to be minorly burned when he put the breaker in.

The employee was operating the miner when a piece of rock fell out between the roof bolts. The piece of rock hit him on the right side of the middle of his back. Employee worked till the end of the shift.

The employee was cleaning the #1 belt tailpiece when he fell backwards. He landed on an anchor pin and injured his lower back. He went to the hospital and they diagnosed the injury as a lumbosacral strain.

Employee was trimming the skirt rubber in a tail-piece and cut his left hand with a utiltiy knife. He went to the hospital after work and they glued the cut closed. He returned to work the next day.

EE was climbing down the ladder on the endloader. He slipped off the bottom step, felt burning in lower abdomen. Went to dr. after work and found out he had a hernia.

EE was shoveling the beltline. He hit his head against the mine roof as he was walking along the beltline causing a mild sprain with some swelling to his neck.

EE was shoveling on the belt line, when he turned in an awkward manner. This caused him to pull a muscle in his lower back.

EE was operating the roof bolter, when a piece of rock fell from the top and hit the canopy. This made the EE make a quick and sharp movement that caused a pulled muscle in his lower back.

The employee was knocking in a wedge to set a timber and small piece of wood went into his right eye. Employee stated he was wearing safety glasses but they had slipped down his nose. Employee went to the eye doctor and had the small piece of wood removed from his eye. He then returned back to work.

Employee was using a chain hoist to pull rail on the flat car. The chain hoist came loose and hit him in the mouth breaking off one of his front teeth. Employee continued working.

The employee was installing a canopy jack on the roof bolter. He placed the canopy jack on the front of the roof bolter and the jack slid off and hit him on the right foot and ankle. This caused a bone fracture under his ankle.

A piece of rock popped away from the roof/rib area striking the employee in the leg and knee area causing some swelling.

EE had just finished loading the roof bolter, when a piece of rock fell from the top and struck him across the thighs of both legs, causing some bruising.

The employee was moving some metal in the supply yard and dropped a piece of metal on the middle finger of his left hand causing a deep cut. He went to the hospital and received 12 stitches.

Employee was reaching behind him to pick up a roof bolt and felt something pull in his back.

The injured employee was helping to finish the belt move. He was swinging a timber to knock in a bottom belt stand and twisted his lower back resulting in a bulged disc.

Employee was operating the continuous miner by remote control and a piece of draw rock fell from between roof bolts and hit his upper left leg causing a bruise.

The employee was building a stopping and as he continued to stack the stopping blocks, a block fell from the stopping striking him in the lower right leg and right foot.

The fall was found at 2:00pm, while the section was idle. This report is in regards to the report of a roof fall. The fall was located in the #7 entry, two breaks inby spad #1998 of the 003 mmu. No equipment, ee or things of the like were injured. Also ventilation was not restricted either.

The employee was moving toward the roof bolter when his knees buckled causing a sprain/strain of lower lumbar region.

The fall occured in return inby section in number 6 entry. It was a horse back that was above anchorage. 6'x4' bolts used, glue 5/8' full grouted. No roadway or airway affected. Timbered off.

Employee was loading supplies, bent over and picked up some half headers injuring lower back.

Employee was helping miner man move miner. While pulling cable, the employee bent over to grab a pull rope. Resting his right hand on top of the miner, the miner began to tram causing the employee's right hand to be pinned against the top.

Employee was operating the Mac 8 mantrip when it stopped tramming. When he was checking the batteries one of the battery covers popped off & got battery acid in his eyes.

Closed tool box lid on his left hand.

Employee was hanging curtain standing on an oil can. It tipped over causing him to strain/pull a muscle in his back.

Employee strained lower back while lifting a roll of welding cable.

Pulling cable off of reel and stained lower lumbar region.

Running shuttle car toward face, hit hole and strained his back.

Employee stumbled against a pallet of rockdust injuring his tailbone area.

Employee was sitting in mantrip when the brakes malfunctioned and smashed employee's finger between the seat and batteries.

Manbus was leaving the surface when the brakes malfunctioned and the box containing the SCSRS fell striking employee in the right arm.

Employee was putting the uni-hauler on charge when a piece of rock fell and struck him in the head and back of neck.

In the #6 entry in the face, Employee was pulling on or pull rope attached to the miner cable, straining back.

The roof fall was 15' wide, 40' long and 6' thick. It was bolted with 4' fully grouted resin bolts. This fall is 1 break inby spad #1242 in #6 entry. This fall has been timbered off and will not be cleaned up. The fall is 4000' from the working section.

EMPLOYEE WAS MAKING UP A BOLT FOR THE NEXT HOLE NEAR THE CENTER OF THE ROOF BOLTER WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS & HIT HIM ON THE HEAD, RIGHT SHOULDER & RIGHT LEG.

THIS FALL FELL IN INTERSECTION 12 BRK. OUTBY SECTION. AT SPAD # 1388, #6 ENTRY, WEST SECTION. TIMBER OFF, WE ARE NOT GOING TO CLEAN UP. APPROX. 4'THICK X 20'WIDE X 20'LONG. 4'RESIN WAS USED.

Employee was operating a Mac-8 mantrip when he lost the wheel and it slipped from his hand injuring his wrist.

The ee was cutting a piece of his knee pad when he cut his thumb requiring stitches.

UNLOADING ROOF BOLTS OFF BOLTER. OFF-LOADING ROOF BOLTS - LEG GOT CAUGHT IN MATERIAL, TWISTING RIGHT KNEE.

Fall in the no. 4 entry sometime between Jan. 20 & Jan. 21, 2005. The main power was off and all the men were knocked off. Fall approximately 60' long, 5' high. Bolted with 4' glue bolts; grade 60; 19' wide. Single crack on left rib, slate top. Miner was in #4 when power knocked. Rock on the miner and the retriever was used to pull miner out Saturday.

A piece of rock fell striking the employee's right hand.

Shoveling belt 3 head & 2b belt. Employee hurt back picking up rocks and putting them in the breakthrough.

HE WAS CUTTING BOTTOM ON #2 ENTRY OF D-MAINS WHEN A PIECE OF LOOSE ROCK FLEW FROM THE RIPPER HEAD OF JOY MINER GOING IN HIS LEFT EYE.

HE WAS WORKING ON A BROKE CAT DAD ON JOY MINER IN #7 ENTRY OF D-MAINS WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN THE BOLTS STRIKING HIM ON HIS LEFT SHOULDER PUSHING HIS HEAD AGAINST THE JOY MI NER FRAME CUTTIGN HIS RIGHT CHEEK.

WHILE WORKING ON JOY MINER WITH BROKE CAT HEAD, HE WAS HIT IN LEFT EAR IN #7 ENTRY ON D-MAIN WHEN A PIECE OF LOOSE ROCK FELL BETWEEN ROOF BOLTS.

HE WAS SETTING ON THE RIGHT RIB OF 7 WHEN A PIECE OF DRAW ROCK FELL OUT BETWEEN THE BOLTS AND HIT HIM ON HIS LEFT LEG.

THE IMMEDIATE ROOF IS SOFT SHELL WITH CLAY STEAKS AND WATER

THE IMMEDIATE ROOF IS SOFT SHELL WITH CLAY STREAKS AND WATER.

THE IMMEDIATE ROOF IS SOFT SHELL WITH CLAY STREAKS AND WATER AND HORSEBACK CONDITIONS.

WHILE BOLTING TOP IN THE 2 ENTRY ON CHAINS A LOOSE PEACE OF DRAW ROCK FROM BETWEEN THE BOLTS AND HIT EE ON THE LEFT SIDE OF FOREHEAD AND LEFT SIDE OF FACE JAW AREA A SMALL CUT OVER THE LEFT EY E HAD TO HAVE 4 STITCHES. EE WAS ADVANCING THE BOLTED TO PU UP THE NEXT ROLLER OF BOLTS.

WHILE WORKING AT POWER CENTER IN 5 ENTRY ON C MAINE AT SPARTA COAL CORP. A PIECE OF DRAW ROCK BROKE LOOSE BETWEEN THE BOLTS AND STRUCK EE ON LEFT SIDE OF BACK LEFT SHOULDER AND LEFT SIDE OF FA CE. THE DRAW ROCK WAS APPROX 2 FT LONG, 16" WIDEAND 2 TO 2 1/2" THICK.

HE WAS SERVICING THE CONTINUOUS MINER WHENA PIECE OF SLATE ROOF FELL AND HIT HIM ON HIS LEFT SHOULDER AND UPPER BACK.

THEN WAS A SLIP OR HORSES BACK THAT FALL IN 6-R CROSS-CUT. THE FALL WAS 40'LX 7'HX 19'WIDE.

HE BENT OVER TO MOVE MINER CANBLE AND STOOD UP TOO FAST AND HIT HIS HEAD ON THE MINE ROOF.

HE WAS TAKING A LOAD OF SUPPLIES TO SECTION HIT A HOLE IN BOTTOM. THE SCOOP HIT THE BOTTOM AND HURT HIS LOWER BACK.

THE BOTTOM WAS UNLEVEL IN THE XCUT BETWEEN #1 & #2 ENTRY. THE SHUTTLECAR BOTTOMED OUT AGAINST THE MINE FLOOR.
