WHILE CLIMBING ON LOADER, THE EE SLIPPED ON OIL SPOT, HIT HIS HEAD & FELL TO THE GROUND ON HIS RT. SIDE. THERE WAS KNOT ON THE RT. EYEBROW, HE ALSO COMPLAINED ABOUT HIS RT. SHOULDER & ARM.
IN THE RIGHT BREAKS BETWEEN 3 AND 4 EE HAD HIS HAND IN THE VISE FOR THE HANDS OFF ONT HE ROOF BOLTER, WHEN HE PASSED THE WRONG LEVER AND CLAMPED HIS LEFT HAND.
EE WAS MAKING BELT SPLICE AND WAS LIFTING BELT CLAMP OVER TOP STRUCTURE WHEN RED DEVIL WAS BEING USED TO TIGHTEN BELT. LEFT HAND WAS CAUGHT BETWEEN RAILING AND CLAMP.
Getting off of scoop, stepped down and leg buckled and he fell over on his leg.

THE EE WAS SHOVELING THE TAILPIECE. THE TAILPIECE BEGAN TO SPILL AND AS HE BENT DOWN A PIECE OF COAL HIT HIM IN THE FACE RESULTING IN SIGNIFICANT PAIN TO THE JAW BONE STRUCTURE.

POWER WAS OFF AT MINE, EE TURNED REOMTE SWITCH OFF AND WAS WORKING ON SCRAPER ON #2 HEAD AND WASSTANDING ON #1 BELT. POWER CAME BACK ON AND DAY SHIFT FOREMAN STARTED BELT FROM OUTSIDE AND THRU EE OFF BELT HITTING RIGHT ELBOW ON BACK STOP.
WHEN THE EMPLOYEE WAS REACHING FOR THE REMOTE CONTROL UNIT A PIECE OF DRAWROCK FELL FROM THE ROOF AND STRUCK HIS LEFT HAND. THIS RESULTED IN A LACERATION TO HIS THUMB. THE EMPLOYEE DID NOT NOT IFY OPERATOR OF RECEIVING STITCHES UNTIL 5-9-01.
