WHEN EE WAS DISMOUNTING COAL TRUCK HIS FOOT SLIPPED FROM ATTACHED LADDER. THIS CAUSED HIM TO FALL BACKWARD ONTO THE GROUND. THUS, RESULTING IN CONTUSIONS TO HIS RIGHT SIDE & A FRACTURE TO HIS RIGHT ARM.

DRIVER STATED SOMETHING BROKE ON TRUCK GOING DOWN HILL AND LET TRUCK FREE WHEEL AND BRAKES WOULD NOT STOP TRUCK, SO HE TURNED IT OVER.
AS TRUCK WAS CLIMBING HILL TO DUMP HIS LOAD TRUCK CAME OUT OF GEAR AND DIDN'T HAVE ENOUGH BRAKESTO HOLD ON HILL, DRIVER STATED HE TRIED TO GET IT IN REVERSE BUT COULDN'T, LOST CONTROL AND WENT OVER EMBANKMENT.

IN THE PROCESS OF CLEANING THE WINDOWS ON THE 600 LOADER, OPERATOR STEPPED FROM THE DECK ONTO THE FENDER OF THE MACHINE. DUE TO WEATHER CONDITIONS THE EQUIPMENT SURFACES WERE WET. OPERATOR S F EET SLIPPED RESULTING IN A FALL OFF THE EQUIPMENT. OPERATOR LANDED ON RIGHT SITE WITH ARM UNDER HIM.

EE REPORTED AN INJURY TO HIS LOWER LEFT BACK. EE WAS SUPPORTING THE FENDER OF THE WATER TRUCK SO THE MECHANIC COULD PUT THE BOLTS BACK IN IT.
EMPLOYEE WAS PUSHING DOZER, PUSHING ROCK IN PREVIOUSLY SHOT MATERIAL,TRYING TO CORNER SOME BIG ROCKS OUT OF THE WAY. GOT A LARGE ROCK CAUGHT UNDER THE BLADE-RAISED THE BACK OF THE DOZER-ROCK SLIPPED-REAR OF DOZER ELL BACK TO ROUND-JOLTING HIS LOWER BACK.
MECHANIC EE INSTALLING TRACK PIN IN E-280- EXCAVATOR, PIECE OF METAL SHEARED OFF-IMBEDDED IN LEFT WRIST LACERATION.
