Employee attempted to do a spot weld without properly positioning welding helmet. EE suffered irritation to both eyes. EE was welding on a gravel trailer.

Employee had just turned off oxygen and acetylene in the back of the work truck. Employee stepped back to get down out of the bed of the truck and their left foot slipped off bumper. They tumbled out of back of truck and fell awkwardly on their left leg injuring the left knee. Knee swollen and unable to walk at this time.

Employee was using a small sledge hammer to knock a roller loose so the conveyor belt at crusher could be lowered to remove excess material. Employee missed roller and struck conveyor frame with right hand cutting and fracturing right ring finger.

Employee was using a a cheater pipe ratchet to adjust the belt on the tail pulley at the hopper. Employee was pushing away from EE's body on ratchet when it sprang back and struck EE on the right lower part of EE's face and jaw. EE had a split lip with contusion and some tooth damage. EE received stitches in the lip and went to a dentist.

Empl was helping other empls get an ATV unstuck from mire. Trackhoe was called in. Trackhoe got stuck. Operator stood up to dismount from trackhoe when leg hit lever causing boom to swing to the right striking empl in the lower back area. Empl received bruises from being hit. Has gone to doctor and received pain medication

An inattentive loader operator allowed their 980H Cat end loader to back into an employee's parked vehicle. The operator had been loading the hopper for over an hour without incident when the accident occurred. There were no problems with the brakes on the loader. There was no damage to the loader, however, the 2000 Tacoma pickup suffered extensive damage.

Employee was changing teeth on backhoe bucket. He struck a tooth with a hammer to loosen it and a piece of steel broke off striking him in the forehead above his safety glasses. Small piece of steel lodged under skin. Went to ER where steel was removed and the wound glued shut. No lost time happened on Saturday just before plant shut down.

Employee was dismounting from D6R Caterpillar dozer when his left foot slipped between the track and the blade arm. He tumbled forward injuring his left knee and left wrist. Injuries were diagnosed as a sprained left knee and left wrist. The fall was caused by buildup of material on the dozer blade arm.

Employee was shoveling under feeder hopper. He felt something in his back. He continued working. His back got progressively worse over the next two days until he couldn't come into work. Doctor gave him a shot and a prescription. Employee did not know what doctor's diagnosis was. Presently, (05/24/13) he is back at work & says his back is not completely well but he is working.

EE was lifting crusher shoes out of shipping container. Shoes weigh approx 80 pounds each. He felt a pulling sensation in abdomen. He had a pulled or strained muscle.

Employee was assisting with welding of lagging onto head pulley for a conveyor belt. He was attempting to hold lagging when the welding rod contacted the head pully causing an arc. He was not wearing eye protection at that time because he was not expecting an arc. Employee felt the effects of flash burn to his eyes after he got home from work later that day.

Employee was inspecting the trough ont he stockpile belt. as he was climbing down his right leg swung but striking a wire cable support causing a puncture would. employee received same stitches to close the wound. he would have returned to work the next day but the dr said bp was too high.

EMPLOYEE WAS REPORTING TO WORK AT SCALE HOUSE. EMPLOYEE LOST FOOTING ON BANK BEHIND THE SCALE HOUSE CAUSING HER TO FALL AWKWARDLY ON LEFT ANKLE. LIGAMENTS WERE TORN IN THE LEFT ANKLE WITH SCRAPES AND ABRASIONS ON THE LEFT LEG. EMPLOYEE TRIED TO WORK UNTIL 5/4/04. WILL HAVE DAYS AWAY FROM WORK.
