A SOAP STONE ROCK AND WATER IN TOP LET ROCK SEPARATE BETWEEN BOLTS, LETTING ROCK BREAK LOOSEAND DROPPING ON EDGE OF CANOPY, THEN SLIDING OFFAND HITTING MAN ON BACK, LEFT ARM AND RIGHT LEG. HE WAS ROOF BOLTING AT THIS TIME.
WHILE TRAMMING DOWN S/C ROAD IN #4 ENTRY TO GET LOADED, OPERATOR WAS MAKING LEFT TURN AND A ROCK ROLLED OUT OF RIB INTO DECK AND STRIKING S/C OPERATOR BETWEEN HEAD AND SHOULDER ON RIGHT SIDE A ND PIECE OF ROCK LANDED ON HIS RIGHT LEG.
AN AREA BETWEEN 5 BELT LINE AND MANTRIP ROAD AT 81 BLK WAS A POTTED PLACE THAT WAS PIERED ON BELT SIDE AND TIMBERED ON MANTRIP ROAD SIDE PROGRESSED IN TIME TO A REPORTABLE FALL.

THE FALL WAS IN AN AREA WHERE TOP WAS UNDERWATER. WHEN THE WATER WAS PUMP DOWN AND THE TOP DRIED IT DROPPED OUT. A SLIP WAS RUNNING ACROSS TEH HDG ALSO. 16W X 6'HIGH X 18 LONG.

#3 MAINS, INTERSECTION OF SPAD 882 CEMENT PIERS HAD BEEN SET TO REDUCE AREA TO ABOUT 10' WIDE. SOME9' BOLTS HAD BEEN INSTALLED. APPROX 20'L, 5-6'H. A LOT OF THE FALL FELL OUT BETWEEN BOLTS.

DUE TO FAILURE OF A PUMP & SUPPORT WALL THE SUMPFILLED UP & OVER RAN DOWN TO SECTION. SINCE WATER WAS RUNNING BY P.C. THE SECTION BOSS MOVED THE POWER CENTER. THE BOLTER HAD ITS TRS UP AGAINST THE ROOF MAKING IT UNMOVEABLE WHEN POWER WAS RESTORED THE WATER ON THE BOLTER.

WAS IN INTERSECTION 4X3 ROOF BOLTING & A ROCK FELL OUT AS HE WAS PUTTING A BOLT IN & A ROCK FELL OUT AND HIT HIS LEFT HAND & WRIST WHILE HE WAS PUTTING A BOLT UP IN.
HE HAD PARKED HIS SC TO HELP HAND MINER CABLE. HE HAD TAKEN HIS GLASSES OFF TO CLEAN THEM WHEN ANOTHER SC OPERATOR ASKED FOR A HAND WITH CABLE. HE THREW HIS GLASSES INTO HIS SHIRT AND GRABBED AHOLD OF CABLE.
2 RIGHT SECTION. 4 WAY INTERSECTION. FALL WAS IN A AREA WHERE ADVERSE CONDITION HAD BEEN ENCOUNTERED WHEN FIRST MINED. WHILE IN PREPARATION. FOR A BELT MOVE THE INTER SECTION STARTED WORKING B EFORE ANY ADDITIONAL SUPPORT COULD BE INSTALLED.THE INTERSECTION FELL IN.
WAS RIDING ON #5 JITNEY & STOPPED AT #2PC TO CHECK BRAKES WHEN HE WENT TO GET OFF OF #5 JITNEY, HIS FOOT WAS ALONG THE SIDE & JITNEY SLID DOWN INTO A BIG HOLE & BRUISED HIS RIGHT FOOT ALONG THE RIB
BOLTER OPERATOR WAS REMOVING DRILL STEEL FROM A FINISHED HOLE HE HAD DRILLED, WHEN A ROCK THAT HE HAD DRILLED THRU SLID DOWN AROUND DRILL STEELAND HIT HIS LEFT THUMB AGAINST THE DRILL HEAD.
