HE WAS STEAM CLEANING EQUIPMENT AND THE H OSE ON THE STEAM CLEANER BROKE AND SPRAYED HOT WATER ON HIS STOMACH.
HE WAS GRINDING ON A CRUSHER ROLLS SCREEN WHERE IT HAD BEEN WELDED AND A CHIP OF METAL SLAG GOT IN HIS EYE.

HE WAS CARRYING TOOLS & CRAWLING UP * DOWN A LADDER ATTACHED TO A CRUSHER SCREEN. THE LADDER WAS WET & MUDDY & HE SLIPPED & FELL 2' BACKWARDS HITTING HIS HEAD ON A CONVEYOR & CABLE.
REPAIRING CONVEYOR BELT ON CONE CRUSHER, LOST HIS FOOTING AND FELL 8 FT TO THE GROUND.

FEEDERSHACK STAND FELL OFF OF CRUSHER UNIT, PINNING EE INSIDE THE CABIN. HE HAD A BROKEN RIB, CUTS, BRUISES & PULLED MUSCLES & LIGAMENTS.
THE V-BELT CAME OFF THE CRUSHER CONVEYOR SO EE RAN UNDER THE SCREEN TO PUT IT BACK ON. ANOTHER SCREEN WAS TIED ONTO THE CATWALK ABOVE WHERE HE WAS WORKING WITH BELT AND AS THE CRUSHER WAS BEIN G SHUT DOWN IT VIBRATED LOOSE, FELL OFF THE CATWALK AND LANDED ON HIS NECK AND BACK CUTTING HIS BACK.

BRACKETS HOLDING THE FEEDERSHACK ONTO THE CRUSHER FEEDER CAME LOOSE AND FELL OVER. HE WAS INSIDE OF THE FEEDER SHACK AND DISLOCATED HIS RIGHT SHOULDER WHEN IT FELL TO THE GROUND.
HE WAS LOADING ALUMINUM TUBING INTO A PICKUP AND CUT HIS FINGER ON THE EDGE.
HE WAS STANDING ON THE CRUSHER SCREEN HEATING A RING WITH A TORCH. ANOTHER EMPLOYEE WAS FREEZING A SHAFT WITH PROPANE FOR THE RING AND SHAFT TO FIT TOGETHER. A THIRD EMPLOYEE CAME UP BEHIND TH EM AND ASKED A QUESTION. INJURED STOOD UP AND TURNED AROUND WITH THE TORCH AND IT HIT THE PROPANE AND EVERYTHING CAUGHT FIRE.
HE WAS HELPING TEAR DOWN A PART OF THE CRUSHER, BENT OVER AND A HOOK ON THE CONVEYOR FELL AND HIT HIM JUST ABOVE THE EYE.
