Seubert Excavators Inc · Fall from machine

The employee was tasked with helping fill water tank. EE was found about 30 minutes later laying face down saying EE was hurt and needed help. At this point it appears that the weight of the trailer shifted off the cribbing under the landing gears and caused the trailer to overturn, either striking against or rolling over the miner.

Seubert Excavators Inc · Struck by falling object

Employee was letting down an electrical cord hanger brace. Brace slipped out of employee's hands and the whole contraption fell on EE's leg, right above the ankle. Accident resulted in a bone fracture in lower right leg.

Seubert Excavators Inc · Struck by falling object

Miner was shoveling on the ground under a cone crusher unit when a rock fell out of the cone crusher and hit the miner on top of EE's hard hat. The miner was brought to the hospital to be examined in worries of a Concussion. Miner was examined and released back to work the same day.

Seubert Excavators Inc · Caught in, under or between a moving and a stationary object

Employee was helping tear down a conveyor during mobilization of the plant. The telescoping conveyor leg tubing hit employee's hand against the frame of the conveyor. This in turn smashed the employee's right pinky.

Employee was cutting a rubber skirt board for a crusher unit conveyor when then knife slipped and cut employee's inner thigh on employee's leg. Received 3 stitches and was back to work the same day.
