Laredo Paving Inc · Caught in, under or between running or meshing objects

While two employees were working around a tail pulley of a conveyor belt, there was miscommunication between the guys on the ground and the plant operator. This resulted in the belt turning on and grabbing one of the employee's sleeves, pulling them into the tail pulley.
