ON FRIDAY 3-24-95 AT APPROX. 3:45 PM, WHILE TRYING TO REPLACE CONVEYOR BOLT ON CRUSHER, HE WAS ON TOP CHUTE HOLDING CHAIN WITH VISE GRIPS. WHEN HE PULLED ON CHAIN WITH GRIPS, THEY UNSNAPPED AN D CAUSED HIM TO FALL BACK, ABOUT 10 FEET TO GROUND.

WE WERE PULLING TRAILER WITH WELDING EQUIP. WHEN THE HITCH UNHOOKED FROM TRUCK. TRAILER GOT STUCK UNDER BUMPER OF TRUCK. HE GOT ON TOP OF BACK OF TRAILER AND TRUCK BUMPER. TRAILER SLIPPED AND WENT BACK UNDERNEATH TRUCK BUMPER AND CAUGHT HIS LEFT FOOT.
