Cornejo & Sons LLC · Struck against a moving object

Employee was driving the haul truck back to the pit when EE anticipated a haul truck coming in their direction. EE applied the brakes and slid the haul truck into the dry drainage ditch which caused EE's head to bump the top of the cabin. Initially the driver was unharmed, days later a concussion was diagnosed, months later EE's driving was medically restricted due to the incident.

Technician was striking a tie rod ball stud with a hammer to remove it from the socket. As technician struck the ball stud, a small metal fragment broke off of the ball stud and went under face shield embedding in employee's neck. Fragment was removed from local clinic with no complications. No list time.

Employee was using skidloader to clean-up around surge bin and conveyors. Impactor was empty and surge bin was empty. However, a rock emerged on conveyor, danced off, bounced off of surge bin and then landed on employee's leg inside skidloader cab.
