June 23, 2010 UT · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer HANDLING OF MATERIALS
Staker & Parson Companies · Struck against stationary object

While changing screens employee bumped elbow on equipment. Employee did not have any medical symptoms following incident. Employee later had symptoms evolve and sought medical care without notifying employer. Injury did not require medical treatment, until later visit when employee had fluids drained and received Steroid Rx treatment.
