EMPLOYEE WAS OPERATING THE TRACKHOE FOR ROCK CRUSHING, THE PIN HAD FALLEN FROM THE BOOM FROM THE ROCK CRUSHER, EMPLOYEE WAS RETRIEVING THE PIN FROM UNDER CONCRETE PIECE LIFTED THE ONE THE PIN WAS UNDER, AND IN DOING SO DISLODGED ONE PIECE OF CONCRETE ABOVE THAT AND SLID INTO AND CRUSHED THUMB.
