Employee was moving a roller when his foot slipped causing him to jam his knee. The employee continued to work until he was taken off for a non-work related injury to his foot. While off work because of his foot he had surgery to repair a torn meniscus in his knee on February 12, 2014. He is still off recovering from his foot surgery. He is not off work because of his knee injury.

THE DRIVER STATED THAT HE HEARD A POP NOISE UNER THE HOOD, HE THEN STOPPED TO CHECK THE T RUCK, TRUCK WAS ON FIRE UNDER THE HOOD,FLAMES WERE COMING THROUGH THE DASH FIREWALL AREA. SAFETYCREW WAS CALLED AND FIRE WAS CONTAINED:PROBABLE CAUSE:LINE BLOWN SPRAYING OIL IN TURBO.

THE SCAFFOLD BOARD SLIPPED BACKWARDS WHIL E HE WAS SHIFTING HIS WEIGHT TO WELD A PLATE IN THE CORNER. HE FELL THE 4' CATCHING HIS SHOULDERON THE ANGLE HOLDING THE BOARD AFTER LOSING HIS BALANCE.
