1ST NOTIFICATION OF INCIDENT JULY 30, 09 BY THE INDUSTRIAL COMMISSION. THE EMPLOYEE GAVE THREE DIFFERENT DATES OF INJURY. THE EMPLOYEE WAS NOT WORKING ON THOSE DATES. THE EMPLOYEE IS REPRESENTED. WE APPEARED FOR MEDIATION WITH LEGAL COUNCIL. THE DATE REPORTED BY EE & HIS ATTORNEY IS JUNE 4, 2009 (?) FOR THE DATE OF INJURY. CURRENTLY THIS INCIDENT IS UNDER INVESTIGATION.
