While installing a cylinder in a scaling machine, a lifting strap failed. The failed strap section hit the employee in the eye causing a scratch to the cornea. All proper PPE was worn.

A piece of loose material contacted the employee on the right arm and left leg. Required stitches.
Employee rolled their left ankle while getting out of a pickup. The injury became reportable after a doctor placed employee on restrictions on 02/26/2025

A significant rain event caused an inundation of water in the Bethany Mine. Thought to have occurred during the day on 5-21-24. Exact time of the inundation is unknown.

The employee was using a hammer drill at the primary crusher, the drill rotated unexpectedly. This unexpected movement resulted in three fractures to the right hand.
Fall in the tunnel occurred 3/5/23. That same day several MSHA personnel had a call. It was determined the fall didn't need to be reported since the structure was designed for such an event. On 3/21/23 another MSHA inspector and field office supervisor instructed management to submit a 7000-1 for this event.

It was determined the fall didn't need to be reported since the structure was designed for such an event. On 3/21/23 MSHA inspector and Field office supervisor instructed management to submit a 7000-1 for this event.

While riding the bus out of the mine, the employee went in to cardiac arrest. CPR was initiated and continued until EMS arrived, employee was transported to a hospital and was resuscitated.
Employee was at Martin Marietta changing a tire when the bead seat band slipped and pinched a finger. They received 7 stiches and has a follow up in 1 week
While loading a face, a piece of shale fell from the face and struck employee in the back.

Employee was loading bolts onto the Fletcher bolting machine. EE was pushing a bolt carousel to move it to continue loading bolts. A bolt slipped out of the clip causing the inside employee's right forearm to contact the corner of the clip resulting in a laceration that required 4 stitches.
