Operator was informed that medical treatment was provided on 6/6/2014 for injuries sustained to the shoulder when employee slipped on ice. The employee no longer works at this mine.

The operator was notified on June 3, 2013 that the employee had developed coal worker's pneumonicosis while he was employed at at Pocahontas Mine 46-09154. On 09/06/2013, the employee was transferred to Grassy Creek Mine 46-08365 as an outby general laborer.

On Friday, 11/15/2013 the mine operator was notified that the former employee was diagnosed with simple pneumoconiosis, category p/q, 1/0. The employee ceased employment on 07/09/2013 due to retirement.

Employee had exited out of his truck to perform a post shift vehicle inspection. He stepped on the edge of pavement and gravel meet; his ankle twisted. X-rays revealed a fracture of the left ankle. NOTE: THIS ACCIDENT WAS ATTEMPTED TO BE SUBMITTED TO MSHA ON 03/11/2013. THERE WERE SOME PROBLEMS WITH THE NETWORK AND THE ACCIDENT REPORT DID NOT GO THROUGH.

Employee recycled the tram pedal and scoop moved forward striking rib, a rock fell striking employee on his little finger of left hand smashing it against scoop door. The little finger was damaged to the extent that it has to be amputated

Employee was in the process of getting the bendictor cleaned at eh #9 head. He shut the #8 belt off by the switch. He the got into the rock box, at which time the belt started again with him on it. Employee grabbed a belt chain which threw him off the belt. He was transported by ambulance to Summersville Hospital, received stitches and treatment for fractured ribs.

EE stated that he was getting out of the scoop when his foot became entangled in the control levers and he twisted his right knee. EE has not missed any work however on 7/1/10 MRI results revealed a torn miniscus and an operation was sch. for 8/13/10. EE had a pre sch. vacation planned and will return on 8/23/10.

EE stated that the bolter was moving from the face area to load supplies onto the bolter. As the bolter was moving, EE was attempting to move the cable out of the way, when the track mounted bolter pivoted on a rise in the bottom and the deck end of the bolter came down on his R foot.

EE had stopped to help the motorman move a sc tire that had vibrated off the flatcar and fell onto the track. While moving the tire he slipped and fell and the sc tire fell on his right leg. EE worked from 7/10/09 until 8/19/09 when he had surgery.

On 1/7/09 EE was in the process of making a belt splice and while using a knife to cut the corner off the belt, EE cut his little finger on his right hand. first aid was done and EE went for medical treatment after completing his shift.

EE was operating the continuous miner cutting coal, when a piece of one of the miner bits broke off and came back and struck EE in the left side of his face. First aid was administered and EE was transported to the hospital where he received 5 stiches.

On 9/22/08, EE was tightening the wheel bolts on a shuttle car. While tightening one of the bolts, the socket slipped off, casing EE to lose his balance and fall on and twist his right ankle and foot. EE was treated and diagnosed with a sprain and returned to work. On 9/27/08, EE was re-examined by another doctor and diagnosed with a fracture.

An unintentional roof fall occurred on #13 belt BK3, near spad #9883. The fall was above anchorage n the neutral entry. Support being used was 4' resin bolt. The area had cribs set prior to the fall, it fell between the cribs that were set. Approx length 25', width 20' and thickness 6 1/2-7'. No injuries occurred.

On 8/6/08 EE was adjusting brakes and checking fluid levels on a mantrip at the surface motor barn. Upon completion of the checks, he lost grip on the access panel while lowering it into the closed position and dropped it approx. 18inches where it then struck EE's groin area.

On 7/15/08 EE along with other EE's were in the process of folsing belt for a belt move when the roof bolt eye broke causing the sheave wheel to swing down and hit EE in the right arm. Sheave wheel was being ised to assist in the positioning of the belt and was attached to the plate eye w/clevis.

EE continued to have pain in his shoulder & went to his family doctor on 1/10/08. His doctor diagnosed him with strained ligaments & took him off work for physical therapy.

EE was operating a scoop, traveled 10 breaks outby the section to gather supplies. The road conditions were very steep, muddy and slippery. While traveling down the hill, his scoop slid into the rib, entrapping his head between the rib and the operators deck. The section electrician traveled outby to work on a piece of equipment and found the employee.

FOOT CAUSING A CHIPPED BONE AT END OF HIS LITTLE TOE.
