Grassy Creek No 1 Coal

Dropping miner cable. Reached to cut rope hanger with knife in hand. Cable hanger broke and cable came down hitting left hand with knife in it, pushed knife into right index finger. Received 6 stitches

The operator was notified on June 3, 2013 that the employee had developed coal worker's pneumonicosis while he was employed at at Pocahontas Mine 46-09154. On 09/06/2013, the employee was transferred to Grassy Creek Mine 46-08365 as an outby general laborer.

Loading 4'water line on motor. Tried to caught end of water line falling off motor. Pulled something in right bicep. Continued working until 11/4/2013 surgery needed.

Trying to remove pin from rub rail on miner. Using a racket extension between pin and floor to push pin out. Holding pin with left hand while raising miner heads, caught and mashed finger between extension and frame of miner.

Carpal Tunnel

On Friday, 11/15/2013 the mine operator was notified that the former employee was diagnosed with simple pneumoconiosis, category p/q, 1/0. The employee ceased employment on 07/09/2013 due to retirement.

Roof fall above anchorage

Pillar fall occurred and a mobile roof support became fouled to where it could not move on it's means or other means available.

Employee had exited out of his truck to perform a post shift vehicle inspection. He stepped on the edge of pavement and gravel meet; his ankle twisted. X-rays revealed a fracture of the left ankle. NOTE: THIS ACCIDENT WAS ATTEMPTED TO BE SUBMITTED TO MSHA ON 03/11/2013. THERE WERE SOME PROBLEMS WITH THE NETWORK AND THE ACCIDENT REPORT DID NOT GO THROUGH.

Employee got thumb caught between a crib block and miner frame while placing a block under frame of miner.

employee was removing a universal joint from a drive shaft. while driving the old caps out with a hammer when a piece of the end of the hammer broke off and flew into wrist.

dismounting coal loader using 3 points of contact, missed or slipped on last step and fell to ground landing on left elbow, employee was transported to Summersville Regional Hospital, x-rays reveled two broken bones in lower arm

Slipped on water line and grabbed on "rock box", laceration on left palm, transported to SMH and received 3 stitches.

Employee, scoop operator, was rock dusting in #6 entry, operator had his hand on the top of scoop door when scoop quit tramming. Employee recycled the tram pedal and scoop moved forward striking rib, a rock fell striking employee on his little finger of left hand smashing it against scoop door. The little finger was damaged to the extent that it has to be amputated

Employee was unhooking trailer from truck. He was unhooking the latch and the hitch came up smashing his finger. He went to Summersville Hospital and received 7 stitches.

Unloading rails from car, rails become lodged on end of car, used chain hoist and clevis attached to a roof plate, eye in roof plate broke causing rails to fall, one of the rails hit employee on top of foot resulting in a fracture to right foot.

employee was setting a rib bolt and a piece of head coal fell and hit him on the right index finger. Employee was transported to Summersville Regional Hospital where x-rays reveled a small hairline fracture to tip or right index finger

Employee was in the process of getting the bendictor cleaned at eh #9 head. He shut the #8 belt off by the switch. He the got into the rock box, at which time the belt started again with him on it. Employee grabbed a belt chain which threw him off the belt. He was transported by ambulance to Summersville Hospital, received stitches and treatment for fractured ribs.

Drilling a bolt hole when a piece of rock 12" x 12" x 8" thick fell and hit employee on right foot behind metatarsal protection. X-rays were taken at Summersville Regional Hospital and revealed a broken bone on top of foot.

Using a hammer on a tire hub, a piece of metal broke off and was embedded in the employee's little finger on right hand. He was taken to Summersville Regional Hospital and had the metal removed. The incision required 2 stitches.

Slipped on steps of coal truck while exiting the cab. Fell to ground striking right elbow and back. Transported to Summersville Regional Hospital. No witnesses to the accident.

Unplanned roof fall on the OO2 unit #24 belt outby spad #13145 and inby spad #13400 fall is approx 46' long and 19'5" wide and 6' to 8' high

Setting spads and cut his finger on a roof bolt plate.

Cutting banding off of roof straps when EE cut banding it sprung back cutting his right forearm requiring 9 stitches.

Cutting mine belt when knife slipped, cutting right knee, the cut required 5 stitches.

EE stated that he was standing behind rockduster when the #2 mantrip came in behind and caught his right ankle between the mantrip and the rockduster.

EE stated that he was getting out of the scoop when his foot became entangled in the control levers and he twisted his right knee. EE has not missed any work however on 7/1/10 MRI results revealed a torn miniscus and an operation was sch. for 8/13/10. EE had a pre sch. vacation planned and will return on 8/23/10.

EE stated that he was placing a metal strap on the bolter to advance to the next row of bolts when his thumb slid through the hole of the strap which had a barb that cut through his glove. This resulted in a laceration at the end of his thumb.

Employee stated that he was helping to change batteries on the #1 scoop. The lid came loose from the open position and fell striking right index finger.

Employee stated that while bolting in the #6 entry a thin piece of rock slid down the drill steel and broke, hitting him in the left eye.

Unintentional roof fall. Fall was 5' to 6' high 20' wide 33' long.

EE stated that he was walking by a parked mantrip and the mantrip battery lid fell and struck his fingers, had hand on mantrip for support.

While walking out the primary escape way ee was crossing a overcast at 21 head when he tripped and fell.

EE stated that the bolter was moving from the face area to load supplies onto the bolter. As the bolter was moving, EE was attempting to move the cable out of the way, when the track mounted bolter pivoted on a rise in the bottom and the deck end of the bolter came down on his R foot.

While putting jack walker back in, motor slipped smashing finger. Required 5 stitches.

EE was helping the section electrician service the right continuous miner, when a piece of rock fell out between the rib bolt and the inside bolt, striking him on the right side of his head, neck and arm.

The roof fall occurred at spad #131 return air course the fall was 0' to 8' high 60'long 18'wide

EE had stopped to help the motorman move a sc tire that had vibrated off the flatcar and fell onto the track. While moving the tire he slipped and fell and the sc tire fell on his right leg. EE worked from 7/10/09 until 8/19/09 when he had surgery.

EE was loading bolts on bolter when he went to pull a metal band out from under bolts and it cut his palm.

Employee stated that a co-worker had picked up a track rail with the forklift andhe rail slipped off the forks and fell on his right foot.

Member was prying up on a shuttle car tire using a slate bar. The slate bar slipped and the shuttle car tire fell on members left leg. Note: EE worked until 5/22/09 and completed his shift.

Unplanned roof fall in the neutral entry. Length-88', width-17', height-6'. Bolted with 4' fully grouted resin bolts. The fall was on old #12 belt inby spad #8237, 7000' from active face.

EE stated that he had climbed onto the miner to hang a reflector, as he was getting off the miner, he felt pain in his lower back. Note: EE stated that the pain started when he went from a standing position to kneeling, to get off miner.

EE stated that he was loading a roll of cable in a mantrip to take underground and mashed his little finger between a roll of cable that was in the mantrip.

Employee was hanging a chain hoist to hold the belt head roller and stepped off of an 8" H-beam and twisted his right ankle.

Unintentional roof fall. Fall was 50' long, 18' wide and 6' high. Type of support: 4' resins bolts. # 2 entry (Neutral Ent) 1 Bk outby Spad # 10644.

An unintentional roof fall occurred on #9 belt #2 Ent (Neutral) inby spad # 10235. Support being used was 4' resin bolt. Approx length 20', width 20' and height 5 1/2' No injuries occurred.

On 1/7/09 EE was in the process of making a belt splice and while using a knife to cut the corner off the belt, EE cut his little finger on his right hand. first aid was done and EE went for medical treatment after completing his shift.

EE was operating the continuous miner cutting coal, when a piece of one of the miner bits broke off and came back and struck EE in the left side of his face. First aid was administered and EE was transported to the hospital where he received 5 stiches.

On 9/22/08, EE was tightening the wheel bolts on a shuttle car. While tightening one of the bolts, the socket slipped off, casing EE to lose his balance and fall on and twist his right ankle and foot. EE was treated and diagnosed with a sprain and returned to work. On 9/27/08, EE was re-examined by another doctor and diagnosed with a fracture.

An unintentional roof fall occurred on #13 belt BK3, near spad #9883. The fall was above anchorage n the neutral entry. Support being used was 4' resin bolt. The area had cribs set prior to the fall, it fell between the cribs that were set. Approx length 25', width 20' and thickness 6 1/2-7'. No injuries occurred.

An unintentional roof fall occurred on #10 belt, bk 7, neutral entry, spad #7406. Approx length 20 ft, width 20 ft, thickness 4 ft. Support being used 4 ft resin bolt. No injuries occurred.

On 8/6/08 EE was adjusting brakes and checking fluid levels on a mantrip at the surface motor barn. Upon completion of the checks, he lost grip on the access panel while lowering it into the closed position and dropped it approx. 18inches where it then struck EE's groin area.

Employee was loading belt structure on scoop. When he set the piece of structure down on his right index finger, it was mashed between two pieces of structure.

On 7/15/08 EE along with other EE's were in the process of folsing belt for a belt move when the roof bolt eye broke causing the sheave wheel to swing down and hit EE in the right arm. Sheave wheel was being ised to assist in the positioning of the belt and was attached to the plate eye w/clevis.

An unintentional roof fall occurred on 2 Unit #8 Entry Spad no. outby 1 break 10095. Length approx. 40', width 20', height 4' - 6'. Support being used 4' resin grouted bolt. No injuries occurred.

EE stated he was in the process of unloading belt structure out of the scoop bucket, he entered the scoop bucket after scoop had been stopped and shut off to get a piece of structure and stepped back out of bucket. When scoop operator let bucket down it hit on his left foot.

An unintentional roof fall occured on BK 20 #2 panel 1 unit. spad #8058 BK20 #ent, lenght 25', width 20', Height 61/2'. Fall occured 78001 from the face. no injuries occured.

An unintentional roof fall occurred on No. 7 belt no. 6 entry, break 15 on spad #7065. This was an old fall reported before. Fall measured 10' long, 18' wide, 3' thick. No injuries occurred. Fall occurred 1600' from the face.

An unintentional roof fall occurred on No. 10 belt, No. 1 entry, spad No. 8955. The fall measured 40' length, 20' width, and 5' in height. No injuries, no passenge of men were affected.

EE was removing the bolter cuthead. After removing the cuthead, ee was trying to untangle the cable and tripped over gob. He caught himself with his left arm straining his shoulder. EE continued to have pain in his shoulder & went to his family doctor on 1/10/08. His doctor diagnosed him with strained ligaments & took him off work for physical therapy.

EE was operating a s/c 10. He pulled behind the miner and the boom of the miner was not elevated high enough. The shuttle car hit the boom of the miner causing employee to strike his mouth into the canopy post.

EE stated that he was helping unload tools at the no.5 belt head when water was turned on at valve where it hooks to main waterline. When line was pressured the nozzle hit him causing a laceration to his ear.

Employee was hanging up fly pads. He went to hit a nail with his pick hammer. The pick hammer was turned backwards and it punctured his left hand.

Unintentional roof fall occurred at Grassy Creek No 1. The fall occurred on No 7 belt, in No 7 entry, break No 9, spad No 7003. The fall measured approx 20' long x 20' wide x 6' high. No injuries occurred. Fall happened 5,000' from the working face.

An unintentional roof fall occurred at Grassy Creek No. 1. The fall happened on No. 7 belt, No. 8 entry, 1 break outby spad no. 7041. The fall measured 20' long, 20' wide & 4' high. The fall does not impede ventilation or escapeway.

EE had finished drilling a test hole. As he was bringing steel out of the hole, the middle extension bounced off the drill head upward, striking EE under the chin and resulted in a 1" abrasion.

EE was operating a scoop, traveled 10 breaks outby the section to gather supplies. The road conditions were very steep, muddy and slippery. While traveling down the hill, his scoop slid into the rib, entrapping his head between the rib and the operators deck. The section electrician traveled outby to work on a piece of equipment and found the employee.

EE was running the No. 1 Shuttle Car. After getting loaded, he was traveling back to the feeder. While making the last turn to the feeder, he had hold of the handrail with his left hand. The c anopy side of the S/C rubbed the rib smashing his left hand. He was taken outside where he was air-lifted to Charleston. EMT gave first aid to him on the section.

EE was cleaning out under a bottom roller with a slate bar. The slate bar was grabbed by the belt smashing his ring finger.

A roof fall above the anchorage zone occurred in the return inby spad 8891, break #16 on 10 belt. The fall was discovered miner at 12:35pm on August 8, 2007. The area will be timbered off. It does not impede travel or ventilation.

An unintentional roof fall occured on the #8 entry on #16 break on 11 belt 60'outby 8029 spad. There were no injuries or equip damage due to fall. Fall measured approx 15'l x 12'w x 0-6' hi. Area was dangered off to prevent travel.

Unplanned roof fall occurred on Break #6 on #10 belt 2 bks inby spad #7379. No injuries or equipment damage due to roof fall. Roof fall measured approximately 25 ft long, 20 ft wide, varied in height from 0 to 6 feet. Area was barricaded off to prevent travel.

EE was running shuttle car, ran shuttle car into rib, jarring neck into canopy.

An unplanned roof fall occurred on the #7 belt at #9 entry and spad no. 7264 outby one break. The length of fall 60', width 18', height 4.5' - 5.5'.

An unintentional roof fall was discovered in #2 section, #6 entry, inby spad #8572. Length 42', width 18', height 5'. Supports being used 4' fully grouted bolts.

Roof fall #6 entry #7 belt. Bolt type 4' resin. Spad # 7026 20' inby. 18' long, 18' wide and 5' high.

Mantrip moving from parking area to staging area caught charging cable and tripped employee backwards, twisting his ankle and landed all his weight down on his foot.

On 12/4/06, EE was operating the shuttle car, tramming from #5 entry to #5 left. EE turned the corner into #5 left and the shuttle car slid into the rib because of the grade of the bottom. The canopy of the shuttle car struck the rib, allowing a piece of coal to enter the operators car.

An unplanned roof fall occurred on the #2 entry on #10 belt and spad #7911. The length of fall was 45', width 18', height 4.5'.

Employee put the #2 section forklift on charge at the end of his shift and was walking to the man-trip to ride outside when he slipped and fell. He stated that the bottom was uneven and he stepped in a hole. X-rays show spiral fractures of the tibia and fibula bones.

Employee was cutting down miner cable, cutting rope hangers with utility knife when the cable jerked causing him to cut his left arm with utilty knife.

Roof fall on #1 unit, left return air course near spad #8152. Fall measured 25' long X 19' wide X 5' high. 4' resin bolts were in use in this area. Fall is dangered off on all sides so no person can enter affected area.

Roof fall near spad 8154 measuring 25' long x 18' wide x 54" high. The fall occurred on the #1 Unit 002 Section. 4' resin bolts and 6' torque tension bolts are being used in this area. Fall was cleaned up.

EE was looking at the reel on a shuttle car, when the lid fell down, causing him to strike his head on the shuttle car

On August 21,2006, an unplanned roof fall occured ont he #1 section. #8 entry and 4,650' outby section. The length of fall was 50' width 18' height 4 1/2'. Will move return to other entry.

On June 21,2006 an unplanned roof fall occured on the #7 belt. The fall is approximately 4-6' thick measuring approx. 25' long and 25' wide. The area will be cleaned up and bolted.

On May 3, 2006, EE and #1 unit crew were on their way outside. When they got down on #6 belt, the belt 1 fire boss was wrecked and had them blocked. EE was helping put mantrip back on track. EE was bent down helping jack. When he stood up, his knee popped and he experienced sharp pain in his knee.

While running shuttle car through water hole, mud splashed up underneath employee's safety glasses into his right eye.

On 4-5-06 employee was tramming the bolt machine from the #4 entry to the #5 entry. He backed the bolter out of #4 and was turning toward #5. While pivoting the machine with the inch controls, a fly board lying in the fly board rack went against the rib and swung out toward ee hitting him in the left temple area.

On April 3, 2006 Employee stated that he put the drill steel aganst the top and hit rotation. The steel bent and spun and hit his left hand cuasing pain.

EE found an unitentional roof fall out by the feeder of #1 section. Spad# 7283. The fall does not impede travel and all sides of the fall have been dangered off and timbered. The fall measured approximately 60' long x 20' wide and was from 0' to 4' in thickness.

Employee was helping watch cable. Operator was tramming bolter out of #3 entry into #4 entry moving cable out of the way. Operator didn't see employee moving cable and he caught his left foot between bolter and rib.

On 2/9/2006 EE was bolting roof, When steel dropped out of hole mashed left ring finger.

Employee was walking from bolter to find section electrician, approximately 26 feet outby the bolter. A rock fell from the roof striking him on top of the head and giving him a sharp pain in the neck between the #4 & #9 vertebrae. A neck brace was applied.

On 2/15/2006 EE was leveling tail piece, while putting wedges under tail piece it slipped and caught his right thumb.

On december 15,2005EE was splicing miner cable, while stipping miner cable. He cut his little finger on left hand.

Employee was installing a roof bolt when a piece of rock fell on the top of the canopy. The rock busted into several small pieces. A piece approx. 12" x 12" x 1/4" thick landed on his arm causing a small laceration requiring 3 stitches.

EE WAS DRILLING HOLE WITH A 54" STARTER WHICH DUE TO HEIGHT CUT FOR FOR PONY DRIVE HE COULD NOT GET HIS HOLE DEEP ENOUGH FOR A 4" BOLT. HE THEN TOOK STARTED OUT & PUT A FINISHER IN HOLE AND WAS TRYING TO PUT PUSHER & FINISHER TOGETHER WHEN HE BROKE FINISHER AT JOINT CAUSING DRIVER TO HIT HIM IN THE CHIN.

Injured was riding in a vehicle leaving mine property when the vehicle he was in was struck head on by a second vehicle entering the property. This accident happened in a curve.

Injured was helping a co-worker remove a piece of stuck drill steel from the bolter drill pot when a piece of metal struck him in the eye.

Injured was tramming roof bolter into face no.2 entry when a section of roof measuring 25' x 19'x 26" fell on the front of bolter forcing the injured head and shoulder to strike the canpoy in the operator compartment.

INJURED WAS PLASTERING A STOPPING IN THE RIGHT RETURN WHEN THE PLASTER SPLASHED ONTO HIS FOREHEAD JUST OVER HIS RT. EYE, RUNNING DOWN BEHIND HIS SAFETY GLASSES INTO HIS EYE.

AN INITENTIONAL ROOF FALL WAS DISCOVERED AT THE #3 ENTRY AT THE DRIFT MOUTH ON 1-5-05. THE FALL MEASURED 20' LONG X 20' WIDE X 3 TO 4' HIGH. THE AREA WAS SUPPORTED WITH 4' AND 6' FULLEY GROUTED ROOF BOLTS.

Employee was bolting on the off side of the bolter putting up a cable bolt. It stuck going in the hole and came out of the head on the bolter, striking him on the right arm and shoulder.

CREW WAS TAKING FLAT CAR TO THE END OF TRACK TO TIE IT OFF AND UNHOOK FROM THE MANTRIP. THE HIGHLINE SLED WAS LOADED ON THE FLAT CAR, AS THE FLAT CAR WAS MOVING, THE HIGHLINE SLED HIT A RIB BOARD ON THE LEFT SIDE OF THE ENTRY. THE SLED SLID SIDEWAYS AND PINNED EMPLOYEE BETWEEN BELTLINE AND SLED.

EE was pulling slack out of #18 belt so they could pull pin out of splice. He was hooking chain to belt clamp to pull belt with scoop. Operator thought chain was hooked and starting pulling causing chain to smash his finger.

An uniteninal roof fall was discovered 5 breaks inby no. 15 belt head near spad no. 5375 fall measured 20' in length x 20' in width x 5' to 6' in height this area was bolted with 4' and 6' fully grouted roof bolts.

AN UNPLANNED ROOF FALL WAS DISCOVERED IN NO. 1 MINE. THE FALL OCCURRED IN #11 BELT, #6 ENTRY (INTAKE) AND 5 BREAKS OUTBY #12 HEAD. THE SIZE OF THE FALL MEASURED 20' LONG, 20' WIDE, 5' HIGH. THE SUPPORT BEING USED WAS 5' B BOLTS. SPAD 3752.

ON JULY 14, 2004 AN UNPLANNED ROOF FALL WAS DISCOVERED IN GRASSY CREEK NO. 1 MINE. THE FALL OCCURED IN #15 BELT AND SPAD NO. 5379, 1 BREAK INBY. THE SIZE OF THE FALL MEASURED 45 LENGTH 20 WIDTH 5 FT. IN HEIGHT. THE AREA HAD BEEN BOLTED WITH 4 FT RESIN BOLTS.

ON JULY 16, 2004 AN UNPLANNED ROOF FALL WAS DISCOVERED IN GRASSY CREEK NO. 1 MINE. THE FALL OCCURED #17 BELT, SPAD NO. 6017, 1 BREAK OUTBY. THE SIZE OF THE FALL MEASURED 20 X20 LENGTH, 20 FT WIDTH AND 84" HEIGHT. THE AREA HAD BEEN BOLTED WITH 4 FT RESIN BOLTS.

ON JULY 14, 2004 AN UNPLANNED ROOF FALL WAS DISCOVERED IN GRASSY CREEK NO. 1 MINE. THE FALL OCCURED IN #5 BELT SPAD NO. 5426 AND #2 ENTRY. APPROX 50 TO 70 FEET OUTBY. THE SIZE OF THE FALL MEASURED APPROX. 50' LENGTH 20' WIDTH AND 66" HEIGHT. THE AREA HAD BEEN BOLTED WITH 4 FT RESIN BOLTS.

INJURED WAS INSTALLING A ROOF BOLT WHEN A PIECE OF SLATE ROOF FELL CUTTING HIS WRIST.

INJURED WAS INSTALLING A ROOF BOLT WHEN A PIECE OF SLATE ROOF BROKE LOOSE AND STRUCK HIM ABOVE THE LEFT EYE, CUTTING HIM. REQUIRED 4 STITCHES.

On the morning of 3/13/04 an unplanned roof fall was discovered in grassy creek #1 mine. The fall occured in #8 & #9 entries spad #'s 5653 & 5669. The size of the fall measured 70'long, 20'wide & approx. 8-9'high. There were no injuries. All approaches to the fall have been supported.

ROOF FALL; LENGTH 50', WIDTH 18' TO 20', HEIGHT 5' TO 6'. PASSAGE OF MEN EFFECTED. NO ROOF BOLTS USED: 4' RESIN. NO INJURIES.

EE WAS LIFTING A BOTTOM ROLLER AND FELT PAIN IN HIS LOWER BACK. HE DID NOT START MISSING WORK UNTIL 12-22-2003. AN ACCIDENT REPORT WAS NOT FILED WITH COMPANY UNTIL 1-9-2004 ON THIS ACCIDENT.

INJURED WAS IN THE NO 1 ENTRY CHECKING THE TOP WHEN A PIECE OF ROCK FELL FROM BETWEEN THE BOLTS STRIKING ON HIS RIGHT KNEE WHERE A KNEE REPLACEMENT HAD BEEN DONE PREVIOUSLY.

WAS WINDING UP BELT AND DROPPED BAR ON RIGHT FOOT.

AREA OF ROOF MEASURING 85'LONG X 20'WIDE X 2'THICK FELL IN NO.2 ENTRY, AND EXTENDED INTO THE CROSSCUT FOR 45'. THE FALL WAS L SHAPED. THE HEADS OF ROOF BOLTS WERE BROKEN OFF. THE AREA WAS MINE D IN 1994 & 1995.

A AREA OF ROOF MEASURING 20' W BY 30' L BY 5' THICK FELL IN THE NO. 3 ENTRY AT SPAD NO. 5373 AND SPAD OUT BY 5371 ONE BRAKE OUT BY THIS AREA IS 1000' OUT BY THE FACE IN ENTRY NO. 3 THIS AREA WAS MINED IN OCT 2002.

EMPLOYEE WAS HOLDING THE HYDRAULIC DRILL WITH HIS LEFT HAND & REACHING DOWN TO THE MINE FLOOR TO PICK UP A WRENCH WITH HIS RIGHT HAND & FELT A CATCH IN HIS MID TO LOWER BACK AREA.

HAD A ROCK FALL IN RETURN ENTRY. ONE BREAK OUTBY SPAD 2074. APPROX. 5500 FT FROM FACE.

EE WAS WALKING IN THE BELT ENTRY HELPING MOVE CABLES. HE RAN INTO A BELT HANGER AND RECEIVED A CUT ABOVE THE EYE BROW.

THE MINER CABLE AND WATERLINE WERE LOOPED ON RT.SIDE OF NO.3 ENTRY. EE SAW THE BOLTER WAS ABOUT TO GET ON THE CABLE. HE STEPPED UP TO MOVE THE CABLE AND HIS LIGHT CORD CAUGHT ON THE SIDE OF THE BOLTER. THIS TWISTED HIS BODY AND CAUSED HISLT. FOOT TO STEP IN FRONT OF THE TIRE. THE TIRE RAN OVER THE EDGE OF HIS LT. FOOT CAUSING A CHIPPED BONE AT END OF HIS LITTLE TOE.

EE WAS LIFTING/SLIDDING AN EIGHT FOOT TIMBER OUT TO SIDE OF SCOOP BUCKET AND MASHED RIGHT MIDDLE FINGER BETWEEN TIMBER AND TOP OF SCOOP BUCKET.

EE HANDED THE SLEDGE HAMMER ACROSS THE BOOM OF THE CONTINUOUS MINER TO A CO WORKER. A PIECE OF MINE ROOF FELL MASHING HIS ARM AGAINST THE BOOM.

ROOF FALL AT INTERSECTION LENGTH 20 TO 22 FT X WIDTH 20 TO 25 FT X HT 6 FTINTAKE AIRWAY SPAD 13543 (INBY NO. 8 HEAD)

FALL OF ROOF MEASURING FORTY-FIVE FEET LONG, EIGHTEEN TO TWENTY FEET WIDE, AND ZERO TO SEVEN INCHES THICK. FALL DID NOT RESULT IN THE BLOCKING OF THE INTAKE.

EE PLACED A CRIB BLOCK OVER TOP OF A BENT SHUTTLE CAR REAL COVER IN AN ATTEMPT TO STRAIGHTEN IT. HE THEN MOVED SIX TO EIGHT FEET AWAY, AT THIS TIME THE SHUTTLE CAR OPERATOR MOVED THE SHUTTLE C AR TO TRY AND STRAIGHTEN THE COVER. WITH THE MOVEMENT OF THE SHUTTLE CAR THE CRIB CAME LOOSE STRIKING EE ON THE THUMB OF HIS RIGHT HAND.

EE WAS GETTING OUT OF MANTRIP & TOOK A CATCH IN HIS LOWER BACK. PER DOCTOR'S DIAGNOSIS, NO TRAUMA INVOLVED - OCCUPATIONAL DISEASE.

EE IS A ROOF BOLTER OPERATOR. ON MAY 10, 2001 HESTATED HIS BACK CONTINUED TO TIGHTEN UP AS HE WAS ROOF BOLTING & HE NEEDED TO LEAVE THE COAL MINE. HE STATED HIS BACK HAD BEEN BOTHERING HIM FOR TWO OR THREE DAYS PRIOR TO MAY 10. HE COULD NOT RECALL A SPECIFIC INCIDENT CAUSING HIS BACK TO HURT.

EE WAS LIFTING RAIL & STRAINED HIS ARM

EE WAS MOVING DRILL BOOM INWARD. HE RUNS HIS FRONT HAND ONT HE CANOPY POST. AS THE DRILL BOOM SWUNG INWARD THE CANOPY HIT THE MINE ROOF AND HIS RIGHT MIDDLE AND RING FINGER WITH INDTY INT HE S T OR THE CAOPY POST. BEEN MISSING WORK 2-21-01

LED PLACED A ROOF BOLT PLATE ON THE END OF THE ROOF BOLT AND ROCK FELL FROM BETWEEN THE 1ST AND 2ND ROW OF BOLTS STRIKING HIS LEFT HAND AND RIGHT FOREARM.

EE HAD PARKED THE ITZ8 ENDLOADER AND WAS CLIMBING DOWN THE LADDER ON THE SIDE. AS HE PLACED HIS LEFT FOOT ON THE GROUND AND PUT WEIGHT ON THE LEG HE FEELS HE INJURED THE SHIN AREA OF HIS LEFT LEG. BEGAN MISSING WORK 1-21-00.

REHANGING CABLE WHEN NAIL WAS DRIVEN INTO RIB. ROCK FELL LOOSE AND HIT HIS RIGHT FOOT.

EE WAS TRYING TO INSERT THE 24 INCH WRENCH INTO THE DRILL HEAD. AS HE LEANED HE SLIPPED AND FELL INTO AN AREA BETWEEN THE DRILL MAST AND DRILL HEAD. HIS LEG STRUCK THE FEED LEVER RESULTING IN HIM BEING CAUGHT BETWEEN THE DRILL MAST AND DRILL HEAD.

EE WAS GETTING OFF ROOF BOLTER WHEN HE CAUGHT HIS FOOT ON A ROOF BOLT. HE FELL AND HURT HIS LEFT KNEE. BEGAN MISSING WORK MARCH 17, 1999.

DURING EE SHIFT HIS BACK BEGAN HURTING. ACTIVITIES INCLUDED GETTING IN AND OUT OF SCOOP, OPERATING SCOOP AND LAYING BLOCKS.

ROOF BOLTER PLATE FELL DOWN BOLTER & SMASHED FINGER BETWEEN PLATE & WRENCH.

EE WAS PREPARING TO TAPE THE LEADS ON THE CONTINOUS MINER CABLE(WHERE CABLE ENTERS JUNCTION BOX ON MINER). AS HE TOUCHED THE LEADS HE RECEIVED AN ELECTRICAL SHOCK.

EE STEPPED BACK FROM THE PLATFORM AT THE DRILL HEAD TO GET PLATES FOR THE ROOF BOLTS. A ROCK FELL FROM AN AREA BETWEEN THE ROOF BOLTS AND STRUCK THE TOP OF HIS HAND, WHICH WAS RESTING ON THE M ACHINE.

ROOF FALL IN NO.3 ENTRY OF MAINS AT CROSSCUT NO. 66 FALL 20 FT WIDE X 20 FT LENGTH X 7 FT HIGH.

EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE WHO WAS PERFORMING IN A WELDING TASK. HE WAS HOLDING THE METAL FOR THE OTHER PERSON TO WELD. HE RECEIVED FLASH BURNS TO BOTH EYES.

MAN WAS HAULING ROCK PROM SLATE FELL, DROPPED SCOOP BUCKET ON ROCK AT BACK CORNER OF BUCKET, ROCK SHOT OUT HURTING OPERATOR IN NOSE.

EE WAS LAYING 8" WATER LINE WHEN HS SLIPPED WHEN MOVING OUT OF WAY OF WATER SPRAYING AT HIM. HE HIT HIS SIDE ON BELT STRUCTURE WHEN HE FELL.

EMPLOYEE WAS ASSISTING ANOTHER MAN IN MOVING A STEEL BEAM. THE OTHER MAN DROPPED HIS END, CAUSING EE TO SUPPORT THE WEIGHT COMPLETELY.

TRIPPED OVER ROCK THAT WAS SICK. HRT RIGHT WRIST.

EMPLOYEE WAS STEPPING DOWN FROM A FORK LIFT. AS HE REACHED THE GROUND HE STEPPED ON A BLOCK OF WOOD. THE WOOD TURNED OVER CAUSING HIM TO BE IN A STRAIN. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 7-30-96. HIS DIAGNOSIS WAS PULLED MUSCLESIN HIS TESTICLES AND BRUISING.

ROCK SLID OFF HIGHWALL NO 2 ENTRY (BELT) 6' LONG 10' WIDE & 4' THICK 6' DOUBLE ROCK BOLTS HAD BEEN USED

INJURED STATED THAT HE STEPPED OVER THE BOLTER ATRS AND RUN INTO THE CANOPY CUTTING HIS FOREHEAD.

INJURED STATED THAT HE WAS PULLING A 13 HP SUBMERSIBLE PUMP & FELT PAIN IN BOTH GROIN AREAS THIS INJURY DID NOT BECOME REPORTABLE UNTIL EE STARTED MISSING WORK 06-02-95

SCOOP HAD BEEN CLEANING TAILPIECE AND WAS BACKING AWAY FROM TAILPIECE INTO CROSSCUT ONE BLOCK INBY THE TAILPIECE. AFTER THE SCOOP CLEARED THE BELT ENTRY THE SHUTTLE CAR STARTED TO TAIL TO DUMP . THE SCOOP KICKED OUT OF TRAM RUNNING BACK IN BELT ENTRY HITTING SHUTTLE CAR. THE CORSSCUT WAS ON GRADE AND DRY.
