Henniker Sand & Gravel Company Inc · Fall onto or against objects

Standing on top of utility body of one ton truck with muddy boots. Tried to jump up but slipped and fell into bed of truck. Fell onto unknown object and punctured lower left back. Object left 1/4 inch hole two inches deep. The object hit and chipped left pelvis and caused some swelling.
