Ee was changing an idler. Ee was on and off of a JLG throughout the job task. The EE went to truck to get a tool to assist with completion of the job. Ee then slipped on ice by the truck and landed on the retractable lanyard that was on EE's back. The injury resulted in 5-7 fractured ribs.

Unloaded packaged blasting agent and then stemmed loaded boreholes. Noticed soreness develop while shoveling.

Employee was using a box cutter to cut rubber to put around a hydraulic hose when the box cutter slipped and struck the employee above the left knee causing a laceration requiring stitches.

The injured employee was helping change screens on the tertiary plant. While starting the nuts on the side rail a second employee on the inside of the screen began to tighten the bolt with a cordless impact wrench and caught the employee's left thumb between the washer and side rail.

Employee states he injured his lower back while washing sand for several days with a water hose preparing the plant for shutdown and then riding in the loader aggravated it. He did not seek medical care until 11/16/2015.

As the EE was repairing electrical components to a conveyor system he needed to travel in a remote area of the quarry. The job took him between numerous tail pulleys and the travel was difficult. As the EE tried to exit the area his foot got stuck which caused him to trip & fall straining/spraining his L knee.
As EE was pulling screen from screen deck he sprain/strained shoulder.
As the EE was helping to change screens the lifting hook came off of the anchorage point and the screen struck the EE on the head. The EE was wearing a hard hat minimizing the injury. This incident occurred on Wednesday June 18, but the EE did not seek medical attention until Friday June, 20 and was placed on modified duty at that time, hence making this a reportable incident.

As the EE was helping to repair a crusher he lifted one end of a pallet that contained machine parts as he was performing this task he sprained his L shoulder. The incident occurred on 9/7/12 but was NOT reported until 9/10/2012.

EE was removing a pin from an access door and got his finger in the hole where the pin was the door opened causing an avulsion on his L index finger.

The Employee was TIGHTENING a HOSE CLAMPS with a SCREWDRIVER and the screwdriver slipped AND PUNCTURE LEFT HAND.

The emp injured works in the quality department and developed tendonitis in his right thumb and wrist due to carrying sample buckets of stone and using wire brush to clean screens for gradations.

EE had upper body thru access door to primary screen sand bin to assist co-work in dislodging massive chunk of sand, ice & snow was frozen to sides of bin overtop of moving conveyor belt. During their attempts, belt caught chunk & suddenly moved it towards ee. Frozen material struck ee and wedged him against access door frame.

WHILE DESCENDING THE STAIRWAY FROM THE PRIMARY CRUSHER, THE EMPLOYEE'S HEEL GOT CAUGHT ON A STEP CAUSING HIM TO LOSE HIS BALANCE AND FALL TO THE BOTTOM OF THE STAIRWAY. HE WAS CARRYING A SOCKE T IN ONE HAND AND A SODA CRACKER IN THE OTHER.
WHILE THE EE WAS DRIVING OUT BROKEN BOLTS FROM THE AC500 CRUSHER, THE 4-POUND HAMMER DEFLECTED OFF OF A BROKEN BOLT AND STRUCK THE BACK OF HIS RIGHT HAND.

WHILE THE EE WAS ASSISTING IN TIGHTENING BELTS ON THE VSI CRUSHER, THE RATCHET ADJUSTER SLIPPED, CAUSING HIM TO SMASH HIS THUMB BETWEEN THE RATCHET AND THE MOTOR MOUNT.
WHILE WASHING OUT THE UNDERSIDE OF CONVEYORS WITH WATER TRUCK, THE EMPLOYEE MISSTEPPED AND TWISTED HIS RIGHT KNEE.
WHILE THE EMPLOYEE WAS PULLING ON THE TAILGATE OF HIS DUMP TRUCK TO RELEASE EXCESS SAND, A LUMP OF FROZEN MATERIAL FELL OUT AND LANDED ON HIS LEFT FOOT.
