The employee was attempting to add a piece of angle iron to the discharge gate to the surge pile feeder. After welding, the gate did not fit properly. He crawled to the gate to investigate. He asked a co-worker for a tape measure. While waiting for the tape measure the material started collapsing around him. As a result the employee was engulfed in the material.

Employee claims to have injured left knee while climbing down from loader. Claim is very suspect, no witnesses and employee has been observed playing football. Investigation is ongoing.

Employee pinched finger while closing Mill door. Received 2 stitches in 3rd finger of left hand.

R knee contusion was suffered when ee caught his foot in 1/4" line climbing down on back air line.
EE was climbing down a ladder inside feeder tunnel #2. He stepped on debris and twisted his knee.
EE was cleaning feeder pans and belt on feeder #3. He attempted to pull himself up and over metal tubing to reach debris and felt a sharp pain in his L shoulder.

He had just gotten off a loader. He took one step and his knee popped. He did not seek medical treatment at the time of incident. He only went to doctor in October.
Employee was loading broken plastic tubes into a loader bucket for disposal. Employee was wearing gloves at the time. A broken peice of tubing pinched the side of his hand and caused injury. Employee received 5 stitches.

HE BACKED TO CLOSE TO THE EDGE OF THE STOCK PILE AND HIS RIGHT REAR WHEEL DROVE OVER THE EDGE CAUSING TH ETRUCK T O ROLL ONTO ITS PASSENGER SIDE. THERE WAS NO BERM AT THIS POINNT. THE BERM COLLAPSED WHEN REMOVED PRODUCT UNDER IT.
