AFTER EVERYONE HAD LEFT FOR THE DAY, FOREMAN NOTICE THAT PIN WAS LOOSE HE PULLED ON THE PIN WHEN IT CLEARED THE HOLE GRAVITY CAUSED THE CYLINDER TO DROP. PINCHED FINGER PIN TIR

ACCIDENT OCCURRED WHIEL ASSEMBLING AN IMPACT CRUSHER. THE SIDES WERE SET ON FLOOR AND THE FEED PLATE WAS HANGING FROM THE OVERHEAD HOIST. WHILE ALIGNING END PLATE WITH SIDES FINGER WAS PINCHED AND CUT OPEN.
