On or about 6/12/12 EE started having pain & swelling in rt hand. Swelling & discomfort led to visit on 7/6/12 w/family dr. He gave EE a wrist guard for support & some anti inflammatory meds. With no positive response from these measures, EE returned to Dr. 7/30/12. Referred to orthopedic dr who determined 8/30/12 that surgery would be performed 9/17/12.

EMPLOYEE WORKED @4 HOURS TRAMMING MATERIAL FROM TEH MUCK KPILE TO THE CURSHER WITH THE 988 CAT LOADER. THE LOADER HAD JUST BEEN MOVED FROM ANOTHER LOCATION. THIS WAS THE FIRST EXTENDED USE AT THE MONTPELIER QUARRY. THE NEXT DAY THE EMPLOYEEHAD A CHICKEN POX LIKE RASH. DR. SAID IT WAS AN ALLERGIC REACTION. NO LOST WORK TIME INVOLVED.

THEY HAD THE FRONT END OF THE CONVEYOR ELEVATED. THEY WERE UNDER THE UNIT TYING UP THE SUPPORT LEGS. THE CHAIN HOLDING THE UNIT EITHER SLIPPED OFF OR CAME UNHOOKED, FALLING.
