On or about 6/12/12 EE started having pain & swelling in rt hand. Swelling & discomfort led to visit on 7/6/12 w/family dr. He gave EE a wrist guard for support & some anti inflammatory meds. With no positive response from these measures, EE returned to Dr. 7/30/12. Referred to orthopedic dr who determined 8/30/12 that surgery would be performed 9/17/12.

THIS WAS THE FIRST EXTENDED USE AT THE MONTPELIER QUARRY. THE NEXT DAY THE EMPLOYEEHAD A CHICKEN POX LIKE RASH. DR. SAID IT WAS AN ALLERGIC REACTION. NO LOST WORK TIME INVOLVED.
