Standing at ground level putting a bar back on grizzly screen. Was trying to guide the bar in to the bolt holes, lost footing and fell back to the ground and the bar fell on employee. Lower back/butt sore.

Ice on ground after snow storm and thaw/freeze. Worker was walking to pick up truck and fell. As EE was falling EE tried to put arm out to catch self which caused a Humerus fracture to right arm.

Installing hydro cones. Wearing gloves, EE put right index and middle finger in between the washer and the cone EE's fingers still between the washer and the cone base were crushed when the impact unexpectedly tightened. EE was treated for a crushing injury to right middle finger and a fracture to the tip of right index finger.
