WORKER WAS APPARENTLY TRYING TO RE-ESTABLISH PRIME ON THE PUMP BY FILLING THE CLEAN-OUT CAVITY WITH WATER. IT APPEARS AS THOUGH WHEN HE LOOSENED THE FIRST BOLT ON THE CLEAN-OUT CAVITY COVER, HOT WATER SPEWED FROM THE CAVITY ONTO HIS FOREARMS, THIGHS AND LOWER FRONT TORSO.

MECHANIC HAD REMOVED THE OLD CUTTING EDGE AND WAS INSTALLING A NEW ONE WHEN IT SLIPPED AND MASHED HIS THUMB BETWEEN THE NEW CUTTING EDGE AND THE PALLET IT WAS DELIVERED ON.
EE WAS OPERATING BOBCAT WHEEL LOADER WHEN HE OVERLOADED THE MACHINE CAUSING IT TO RARE UP ON HIM. WHEN THE MACHINE CAME DOWN, HE FELT A STRAIN IN HIS LOWER BACK.
EE DESCRIBES THAT HE WAS WALKING ACROSS GROUND NEAR CRUSHER BINS AND FELT A JERK IN HIS LOWER BACK. COMPLICATIONS BEGTAN LATER IN THE DAY. THIS REPORT IS LATE DUE TO THE FACT THAT THE EE DID N OT MAKE US AWARE OF THE SITUATION UNTIL HE BEGAN TO MISS WORK SEVERAL DAYS LATER. DR.'S EVALUATION INDICATES A LUMBAR STRAIN.
