WORKER WAS INSTALLING A NEW BEARING IN A REMOVED CONVEYOR ROLLER. HE WAS USING A 4 LB SLEDGE HAMMER AND A 1 7/16" CHROMED SOCKET. AS THE HAMMER STRUCK THE SOCKET, IT TILTED AND THE HAMMER SLID , MASHING THE TIP OF HIS INDEX FINGER ON HIS LEFT HAND. THE TIP OF HIS FINGER WAS BROKEN AND THE UPPER PORTION OF THE FIRST JOINT, INCLUDING THE FINGERNAIL, WAS PEELED OFF THE BONE.
