Miner was kicking a frozen door lock to break it loose

While doing maintenance on trackhoe, EE leaned over the edge of the bucket to check on a pin. EE then fell against the edge of the bucket and received a contusion to his rib.
Worker was cleaning the ground around the screen assembly and a rock fell off the screen which cut him when it hit him in the head. Worker was wearing a hard hat at the time.

EE WAS ATTEMPTING TO FREE A STUCK CONTROL LEVER USING A WRENCH. THE WRENCH SLIPPED AND EE HIT HIS HAND ON THE FRAME OF THE BOBCAT.
WATER TRUCK ROLLED OVER DUE TO TRAVELING OVER UNEVEN GROUND.

PRY BAR SLIPPED WHEN MINER WAS TRYING TO UNJAM THE CONE AND SPRAINED HIS LEFT WRIST. ACCIDENT WAS REPORTED TO COMPANY SAFETY OFFICE ON 5-16-03
