While replacing buckets on the 22 foot elevator employee slipped on loose sand on the ground catching his hand between the bucket and belt twisting his wrist. Diagnosis strained right wrist/Ulna-Carpal Ligament Strain. Ice pack for 30 minutes 3 to 4 times a day.

While dismounting the 966F Cat Rubber tired loader EE twisted his right knee. The treatment was to apply a cold pack for 30 minutes 3 to 4 times a day. No improvement was sent for MRI on 8-6-2013. This showed positive for mensical tear, awaiting authorization for specialist.

Engineer operating rubber tire loader noticed bloodshot left eye and cloudy vision. First doctor left eye abrasion-foreign body, second doctor superficial irritation to cornea. Final visit 06/20/2011 because no doctor on weekend, released with rx prescription but no restrictions 06/16/2011.

This operator took another out to the dredge by jon boat. His return trip was abruptly stopped by a sand bar. He lunged forward and caught his foot in a bottom boat bow, wrenching his back. Sent to doctor for lumbar strain, muscle spasms. Doctor placed him on modified duty. Tough to not reach or stoop or bend on any sand plant job. He stays at home and gets physical therapy.

Operating engineer minimally sprained his right ankle walking down a set of stairs which bridge a conveyor. The doctor told him to put ice on it and gave him OTC medication, scheduled follow-up appt. for 10-23-2009. EE worked his next scheduled shifts without incident. Doctor then prescribed physical therapy at follow-up appt. 3 x a week to last for week and a half.

EE, Engineer, was measuring river dredge on a loboy trailer. He was stepping down and slipped on ice. He notified the Safety Director, who sent him to medical clinic. The dr. diagnosed Lt SIJ Strain (back strain), prescribed a pain medication, ice pak treatments, and Moderate Duty. He scheduled an appointment for 1-27-09. EE returned to work.

Kaw Valley Sand and Gravel, Inc. · Caught in, under or between running or meshing objects

Operator removed guard to clean the steel tire of a rotary sand drying drum. The operator was holding a rag on the tire while it was running. His two finger tips were smashed between the moving steel rollers. KU Med doctors amputated the distal tips of both his ring finger and middle fingers on his left hand. The bandages will be on for at least another week.

Mechanic replacing a cutting edge on a 966 cat rubber tire loader. Held blot head and air wrenched the nut, the bolt spun in his right hand causing a laceration to right index finger. 3cm wide, 7 stitches
