WHEN THE HOOK WAS BEING POSITIONED BY HAND TO HOO K THE ROCK, THE HOOK JAMMED BETWEEN THE ROCK AND THE MOVING SWIG JAW THROWING THE END OF THE HOOK BEING HELD BY HAND AGAINST THE STEEL FEED HOPPER, PINCHING HIS HAND IN BETWEEN. HIS RING FING
EMPLOYEE WAS INSPECTING A TRUCK FOR CRACKS IN ITS LIFT AXLE WHEN HE BUMPED HIS HEAD AGAINST THE UNDERSIDE OF THE TRUCK CAUSING A BRUISE AND A LACERATION TO THE FOREHEAD.THE LACERATION TOOK FOU T STITCHES TO CLOSE.
