EE was operating D8 bulldozer when EE ran over a large rock. The machine was jarred causing dust to fall off of the roof of the cab. A piece of dust got into EE's eye, causing a scratch to EE's eye.
EE was moving material from a bench to be loaded onto trucks. EE had a full bucket and swung to the left rapidly, causing the rear to lift. EE tried to correct the instability by swinging back to the right, but the machine was too far off balance causing it to overturn on the cab side. In the process of the rescue the ee pulled a muscle in EE's back.

EE was working on blow bar on crusher. The employee pinned bar into rigging equipment lifted it and began to rotate bar before placing it into position. The blow bar disengaged from the rigging equipment when the other employees were rotating the blow bar. The blow bar bounced off the crusher and landed on the employee's foot causing an ankle fracture and lost time.

EE was carrying a screen, lost balance causing right hand to slip at the edge of the screen resulting in a laceration of right little finger.
Employee dismounted aerial lift successfully, started walking towards rear of machine and EE lost balance, slipped and fell to the ground injuring the right hamstring muscle.

After coming to a stop & looking both ways, EE crossed the road in the water truck. A passenger vehicle traveling at a high rate of speed hit the water truck at the front right tire area. The EE was taken to a health works facility where EE was diagnosed with a contusion and swelling on EE's left wrist.
EE was climbing up a ladder at the classifier when EE heard the right knee pop. The EE was not doing any unnecessary twisting on the ladder while maintaining 3 points of contact. EE indicated did not have leg positioned straight, with EE's foot at a slight angle to the rung when EE applied weight to it.
EE was climbing down the 4 rung folding ladder for conveyor belt 102 when the right foot became pinched between the last rung and a washer used to secure the ladder in an upright position when not in use. The EE fell to the ground landing on right elbow which put strain on right shoulder causing internal tissue damage to the joint.

Two employees were driving the maintenance truck down the ramp to the bottom level of the primary when the brakes went out. The truck was driven into a berm to come to a sudden stop. Both employees showed no injuries at the time of accident. One employee sought medical attention for lower back on 11/5/2019.

A miner was observed collapsing next to his service vehicle while changing his work boots after his shift was completed. EMS was called and transported the miner to a local emergency facility. The miner died of natural causes several days later in the hospital.

MOVING FLIP GATE IN BOX AT HEAD END OF CRUSHER BELT, EMPLOYEE WAS STRETCHING OVER THE BOX TO REACH CHAIN. HE FELT RIGHT KNEE CAP POP. WHEN HE STOOD UP STRAIGHT IT POPPED BACK.

EMPLOYEES FOOT GOT CUAGHT INSIDE OF THE STONE BOX AS HE WAS EXITING FROM THE AREA HE WAS WORKING. HE THEN LOST HIS BALNCE AND FELL ONTO THE CATWALK. HIS ARM GOT CUT BY A CATBAR.

EMPLOYEE WAS LOADING THE TRUCK OUT OF THE 2B BIN WHEN HE PINCHED HIS FINGER BETWEEN THE BIN HANDLE AND THE RAILING.
EMPLOYEE WAS CLEANING OUT WET LIME MIXING SCREW. THE PIECE OF STEEL USED TO CLEAN OUT THE SCREW CAUGHT ON ONE OF THE FLIGHTS AND STRUCK THE EMPLOYEE IN THE CHIN AND RIGHT ARM.

EMPLOYEE WAS USING A JACK HAMMER TO REMOVE MATERIAL FROM CRUSHER PARTS WHEN A PIECE OF MATERIAL STRUCK HIM IN THE RT EYE
