EE was operating D8 bulldozer when EE ran over a large rock. The machine was jarred causing dust to fall off of the roof of the cab. A piece of dust got into EE's eye, causing a scratch to EE's eye.

EE was moving material from a bench to be loaded onto trucks. EE had a full bucket and swung to the left rapidly, causing the rear to lift. EE tried to correct the instability by swinging back to the right, but the machine was too far off balance causing it to overturn on the cab side. In the process of the rescue the ee pulled a muscle in EE's back.

EE was working on blow bar on crusher. The employee pinned bar into rigging equipment lifted it and began to rotate bar before placing it into position. The blow bar disengaged from the rigging equipment when the other employees were rotating the blow bar. The blow bar bounced off the crusher and landed on the employee's foot causing an ankle fracture and lost time.

EE was carrying a screen, lost balance causing right hand to slip at the edge of the screen resulting in a laceration of right little finger.

Employee dismounted aerial lift successfully, started walking towards rear of machine and EE lost balance, slipped and fell to the ground injuring the right hamstring muscle.

After coming to a stop & looking both ways, EE crossed the road in the water truck. A passenger vehicle traveling at a high rate of speed hit the water truck at the front right tire area. The EE was taken to a health works facility where EE was diagnosed with a contusion and swelling on EE's left wrist.

EE was climbing up a ladder at the classifier when EE heard the right knee pop. The EE was not doing any unnecessary twisting on the ladder while maintaining 3 points of contact. EE indicated did not have leg positioned straight, with EE's foot at a slight angle to the rung when EE applied weight to it.

EE was climbing down the 4 rung folding ladder for conveyor belt 102 when the right foot became pinched between the last rung and a washer used to secure the ladder in an upright position when not in use. The EE fell to the ground landing on right elbow which put strain on right shoulder causing internal tissue damage to the joint.

Two employees were driving the maintenance truck down the ramp to the bottom level of the primary when the brakes went out. The truck was driven into a berm to come to a sudden stop. Both employees showed no injuries at the time of accident. One employee sought medical attention for lower back on 11/5/2019.

Employee was removing rust from the #8 chute when some rust landed on EE's eyebrow and safety glasses prior to getting into EE's right eye.

Employee was removing a chain puller from the maintenance truck when it slipped out of his hand and struck him on the right shoulder.

A miner was observed collapsing next to his service vehicle while changing his work boots after his shift was completed. EMS was called and transported the miner to a local emergency facility. The miner died of natural causes several days later in the hospital.

Employee was cutting a conveyor belt when the knife slipped, cutting the back of his left hand. Employee was wearing gloves.

Employee was cutting a piece of rubber during repairs to the #102 screen when the utility knife slipped and cut his left thumb.

EE PARKED THE R-35 RUCLID HAUL TRUCK AND WHILE DESCENDING THE LADDER WITH HIS LUNCH BOX SLIPPED OFF THE 2ND LAST STEP AND FELL TO THE GROUND TWISTING HIS RIGHT KNEE.

EE WAS EXITING TRUCK WHEN HE SLIPPED ON STEP AND FELL BACKWARD ONTO THE CATWALK.

AS EMPLOYEE WAS TRYING TO POKE FREE BUILT UP MATERIAL IN CHUTE WITH PRY BAR IN A HORIZONTAL POSITION THE MATERIAL BROKE FREE CAUSING THE PRY BAR TO FLY UP IM THE AIR CATCHING THE EMPLOYEE IN T HE RIGHT EAR LOBE.

MOVING FLIP GATE IN BOX AT HEAD END OF CRUSHER BELT, EMPLOYEE WAS STRETCHING OVER THE BOX TO REACH CHAIN. HE FELT RIGHT KNEE CAP POP. WHEN HE STOOD UP STRAIGHT IT POPPED BACK.

EMPLOYEE WAS ATTEMPTING TO LIGHT PILOT ON HAUCK BURNER HE WAS STANDING ON PLATFORM HOLDING PROPANE TANK WITH RT HAND ANOTHER EMPLOYEE WAS INSTRUC TED BY EE TO LIGHT THE MAIN BURNER AFTER HE WA S CLEA R THE OTHER EMPLOYEE OBSERVED THE GAUGE FOR THE PILOT LIGHT TO BE IN THE LIT POSITION & AT THIS TIME THE OTHER EMPLOYEE LIT THE MAIN FLAME WITHOUT NOTICING IT EE WAS CLEAR WHEN FLAME I

WHILE EMPLOYEE WAS HOLDING THE BIN HANDLE, A GRADER PASSED UNDER THE BIN. THE ROOF OF THE GRADER HIT THE CLAMGATE CAUSING THE HANDLE TO SWING AND STRIKE THE EMPLOYEE ON THE LEFT SID4E OF HIS HEAD.

EMPLOYEE SLIPPED OFF OF A CATWALK TO THE GROUND. HE STRUCK HIS BACK ON THE CATWALK CORNER WHEN HE FELL. EMPLOYEE WAS STANDING ON BEAM ABOVE CATWALK AND STEPPED DOWN ONTO THE CATWALK WHICH GA VE WAY AND HE LOST HIS BALNCE.

EMPLOYEES FOOT GOT CUAGHT INSIDE OF THE STONE BOX AS HE WAS EXITING FROM THE AREA HE WAS WORKING. HE THEN LOST HIS BALNCE AND FELL ONTO THE CATWALK. HIS ARM GOT CUT BY A CATBAR.

EMPL WAS EXITING BIN TRUCK WHEN HE STEPPED FROM LADDER & SPRAINED RIGHT ANKLE.

EMPLOYEE WAS ASISTING MAINTENANCE MAN TO INSTALL A FRONT LEAF SPRING IN A BIN TRUCK WHEN EMPLOYEE FELT A PAIN IN HIS LEFT SIDE

EMPLOYEE WAS LOADING A BIN TRUCK WHEN HE PULLED ON THE LEVER TO LET THE STONE OUT OF THE BIN AND TWISTED HIS BACK.

EMPLOYEE WAS LOADING THE TRUCK OUT OF THE 2B BIN WHEN HE PINCHED HIS FINGER BETWEEN THE BIN HANDLE AND THE RAILING.

EMPLOYEE WAS CLEANING OUT WET LIME MIXING SCREW. THE PIECE OF STEEL USED TO CLEAN OUT THE SCREW CAUGHT ON ONE OF THE FLIGHTS AND STRUCK THE EMPLOYEE IN THE CHIN AND RIGHT ARM.

EMPLOYEE WAS EXITING FROM TRUCK WHEN HE STEPPED ON SOME FROZEN LIME AND OVERTREAD HIS ANKLE.

EMPLOYEE WALKED BEHIND A RAISED TRUCK BODY TO CLEAN IT OUT. WHILE HE WAS ROUNDING THE CORNER OF THE BODY A STONE WHICH WAS STUCK IN SOME MUD, DISLODGED AND STRUCK EMPLOYEE LEFT FOOT.

EMPLOYEE WAS USING A SLEDGEHAMMER TO INSTALL WEDGES IN A CRUSHER WHEN THE HAMMER SLIPPED AND STRUCK HIM IN THE LEFT FOOT

EMPL WAS ASSISTING WELDER INSTALLING CHUTE LINERS ON SCREEN TWOER WHEN WIND BLEW DUST IN EYE.

EMPLOYEE WAS CLIMBING DOWN THE LOADER LADDER WHEN, HE SLIPPED OFF OF THE LAST STEP AND FELL ON HIS BACK.

EMPLOYEE WAS USING A JACK HAMMER TO REMOVE MATERIAL FROM CRUSHER PARTS WHEN A PIECE OF MATERIAL STRUCK HIM IN THE RT EYE

EMPLOYEE WAS MEASURING LIME IN THE SILOS, WHEN HE STEPPED ON A PIPE TO CROSS OVER IT AND SLIPPED AND FELL ON THE P IPE WITH BOTH KNEES.

EMPL WAS OPERATING BIN TRUCK HAULING MATERIAL TO STOCK PILE. ENTRANCE TO PLANT HAD BUMPY ROADS CAUSING TRUCK TO BOUNCE AROUND.

EMPLOYEE WAS WELDING BRACKETS ON EUCLID QUARRY TRUCK WHILE STANDING ON A STEPLADDER.BOTTOM STEP ON A 4' STEPLADDER BROKE,EMPLOYEE FELL AND LANDED ONM PORTABLE GRINDER LAYING ON THE GROUND.

EMPLOYEE WAS BUILDING A CATWALK IN THE LIME PLANT LOADING AREA WHEN DUST FELL OR BLEW INTO EMPLOYEES LEFT EYE.

EMPLOYEE WAS WALKING AROUND TO SHUTDOWN THE CRUSHING PLANT .EMPLOYEE STEPPED ON A ROCK LOST HIS BALANCE AND STRUCK HIS RIGHT ELBOW ON A CONCRETE PILLAR.

EMP WAS CLIMBING INTO HAUL TRUCK WHEN HE SPRAINDD HIS LEFT WRIST.

WHILE USING A CUTTING TORCH TO CUT A PIECE OF HOT METAL FLEW UNDER CUTTING SHIELD INTO EMPLOYEE'S LEFT EYE.

WHILE USING AIR TO PUT AIR IN TIRES ON SCREEN DECK AIR LINE RUPTURED BLOWING INTO EMPLOYEES FACE EMPLOYEE SIDESTEPPED AND BUMPED LEFT ARM AND LEFT EAR ON HANDRAILING

THE OVERHEAD CR5NE WHICH RIDES ON WHEELS AND AN I BEAM FELL APPROX. 15 FT WHEN PART OF THE CRANE SUPPORT FIXTURE BENT, ALLOWING THE CRANE TO SLIDE OFF THE BEAM PERPENDICULAR TO THE AXIS OF THE BEAM. THE CRAN9 MOTOR AND BLOCK FELL SIDEWAYS OR 5 FT AND DOWN 15 FT.

EMP WAS LIFTING3THE TONGUE OF THE TRAILER, HELPING TO HOOK IT TO A PICKUP TRUCK. AS HE DID THIS HIS BACK CRACKED RESULTING IN TORN LIGAMENTS AND MUSCLES IN L OWER PART OF HIS BACK

EMPLOYEE WAS REQUESTED TO REMOVE GREASE IN VICINITY OF PRIMARY CRUSHER. EMPLOYEE CLIMBED ON CONVEYOR BELT AND SLIPPED ON ICY BELT. EMPLOYEE FELL APPROX. FIVE FEET. EMPLOYEE SUFFERED ONE CRACKE D RIB AND BRUISED LEG AT ANKLE.
