Operator was working on a platform, removing a crusher, when EE stepped in an opening in the floor, EE lost balance and caught self on a railing with rib cage and knee. EE did not report the accident until 9/8/2020
EE was wiping dirt from rear hazard strobe on top of tank during pre trip inspection. While climbing down the ladder, EE's wedding ring became caught on ladder rung causing a deep laceration to ring finger on right hand.

While pushing material on overburden pile, dozer operator observed smoke coming from around bottom of cab. Attempted to extinguish fire with fire extinguisher but ultimately machine was fully engulfed.

A suicide victim was discovered by employee of mine operator at 9:00 AM on 10/19/2011. Victim was located away from active mining near an abandoned out building along with his SUV.
While driving in pin for tooth on loader bucket, an unknown object struck employee in the area around left eye causing laceration and contusion.

WHILE PERFORMING PRE-SHIFT INSPECTION OF CATERPILLAR 990 LOADER, EE TRIPPED OVER ELECTRICAL EXT ENSION CORD TO ENGINE BLOCK HEATER ELEMENT. DURING THE FALL EE STRUCK RIGHT HAND, 5TH FINGER ON COUNTERWEIGHT CAUSING FRACTURE.
LABORER WAS CLEANING ROCKS FROM CONVEYOR #6 WHILE IT WAS OPERATING. THE HOE HE WAS USING CAUGHT THE CONVEYOR BELT JERKING THE HOE TO THE RIGHT BREAKING HIS RIGHT ARM AND THE HOE HANDLE WHEN HE HIT THE STATIONARY ROLLER BRACKET. LABORERS ARE TRAINED NOT TO CLEAN OPERATING EQUIPMENT.

BLOCK-UP ON TOP CHUTE OCCURRED.WHILE EMPLOYEE WAS INVESTIGATING WHAT WAS CAUGHT IN CHUTE,A ROCK SLID OVER THE TOP OF THE CHUTE STRIKING EMPLOYEE IN THE FACE.

THE QUARRY AREA WAS FLOODED WITH WATER DUE TO HEAVY RAIN THE NIGHT BEFORE., HE PUT THE PUMP IN POSITION TO REMOVE WATER AND WHILE TRYING TO START THE GASOLINE PUMP, THE HANDLE CAME OFF ROPE AN D HE SPRAINED LEFT SHOULDER.
WHILE BURNING STEEL WITH ACETYLENE TORCH INSIDE THE OVERSIZE BIN, A HOT PIECE OF SLAG ENTERED HIS SHOE AND BURNED HIS FOOT.
I.NAME WAS ATTEMPTING TO PULL AIR HOSE CONNECTING THE DRILL TO THE COMPRESSOR WHEN HE STRAINED HIS BACK.
