AT APPROX. 9:00 AM DRIVER OBSERVED THAT THERE WAS NO SIGN OF EE WHILE WAITING TO DUMP HIS LOAD INTO THE HOPPER AT THE CRUSHER. DRIVER WENT TO THE CRUSHER BOOTH AND FOUND EE ON THE FLOOR. NOTIF IED THE MGR AND PROVIDED FIRST AID. AMBULANCE ARRIVED AT THE FACILITY AT APPROX. 9:05 AM AND THE ELIZBETHTOWN HOPS. AMBULANCE ARRIVE AT 9:15 AM DEATH AS 11:13 AM.
