A small rock blocked jammed the conveyor belt on a screening plant. When employee hit the kill button to stop the machine, a large chunk of clay rock fell and hit their RIGHT leg. Sustained fractured Tibia and Fibula and broken bones on top of foot.

Employee was riding in the basket of the manlift through the yard and hit a small bump. The employee struck EEs left elbow on the manlift basket cage. The injury started as a first aid incident and was being treated by Ortholive with conservative care. The incident became a reportable incident on 5/20/25 when the employee went to an urgent care facility and was given restrictions.

Employee was cutting a metal plate at the crusher on Wednesday and a piece of hot slag went down EE's welding glove and burnt EE's hand. On the way home from work Thursday EE's hand started hurting and when EE got home decided needed to have it looked at.

EE was installing hosing rubber on a screen and when he cut the rubber, he followed through and cut his left thumb. Has some stitches in thumb. Back to work same day at regular job.

When trying to remove a bushing with a hammer and punch, the hammer hit the edge of the fender and made him miss the punch and hit his left hand. Bruised the left hand near the thumb. He went to the emergency room for x-rays. No broken bone but was given antibiotics. He then came back to work.

Employee got out of the haul truck (175), walked a short distance on the haul road. He started to bend down to pick up a rock & experienced lower back pain before he could touch the rock. The foreman took him to the emergency room at CUPH in Plattsburgh.
EE was helping change a bearing on a head pulley. EE was standing in a chute with angle iron in it, when the wrench slipped and his foot slipped, causing EE to fall and hit his head on the side of the chute, cutting his left eye brow. He went to the emergency room and received two stitches.
Employee developed a headache & felt sick. Claims it was fumes in the cab. The carbon monoxide detector that is mounted in the cab did not go off. The employee was taken to the hospital and was found to have carbon monoxide in his system. He was told to rest for the day. No medication was given.

EMPLOYEE WAS CHIPPING OUT BUILT UP MATERIAL IN A STONE BOX, A CHUNK OF DIRT FELL ABOUT FOUR FEET AND STRUCK THE EMPLOYEE ON THE LEFT SHOULDER & COLLARBONE. IT BRUISED THE SHOULDER & COLLARBONE .
DESCENDED A LADDER IN AN UNSAFE MANWAY (NOT THESE POINTS) CAUGHT HEAL AND FELL. BRUISED SHOULDERON CEMENT PIER.
STANDING ON CONCRETE SLAB THAT HE WAS JACKHAMMER OUT. THE JACK HAMMER FELL OVER AND PINCHED HIS FINGER BETWEEN THE JACK HAMMER AND THE CEMENT.
