Employee was shoveling off the primary belt at the crusher and when EE went through the railing to get onto the belt, EE felt a burning sensation in EE's back. This injury was originally a first aid incident, but on 6/17/25 the employee was taken out of work.

Upstone Materials Inc. · Struck by flying object

EE was adjusting the jaw die setting on the crusher. While EE's right hand was on the rod the hydraulic cylinder became overpressurized and failed on the cylinder end. When the cylinder failed the hydraulic fluid hit EE's hand at a very high pressure. The cylinder failure caused severe bruising and a laceration to EE's right hand and the back of the arm above the wrist.

Employee was turning on water system that feeds the crusher for the season. The system consists of a pump and eight inch PVC piping. When another employee turned on the pump an eight inch tee failed causing water to spray in the direction of the injured employee. The injured employee took off immediately when the pipe failed and fell and twisted left knee.

Rainy, snowy day EE was pushing material on a bench to an excavator that was casting the material off the bench. EE stopped at 10:15 to take a break when EE stepped on the track his foot slipped and he fell. He bruised his shoulder and has a ankle fracture. It was very muddy and slippery and EE is 68 years old.

Loader bucket caught a rock and threw the operator forward in the 980 G seat. When he came back in the seat, he claims to have pinched a testicle. On Thursday, 4/9/09 he went to the emergency room. they said not to work on Friday. He came back on full duty on Monday, one day lost.

He was changing a chute liner, when the wrench slipped and his hand struck the chute liner and cut his little finger. He went to urga-care, they stitched it up and he was back at work in less than one hour. No light duty, no lost time. Reportable because of the stitches.

Ee alledgedly slipped on grease in front of the time clock and fell hurting his back. Was taken by he save to hospital. was checked over and told to go back to work after 48 hrs. ee was under reasonable suspendion for alchol and knew he was going to be tested.
