Upstone Materials Inc. · Caught in, under or between a moving and a stationary object

The miner was backing the haul truck into the shop to be repaired and caught their hand between the truck's catwalk handrail system and the garage door frame. The handrail system was bent, and the employee reached through the cab window to pull it in while backing in. The impact caused a contusion to EE's left hand.

Employee was moving a 10 foot piece of 2X4 box beam and felt something snap in EE's left arm. Employee was evaluated in the ER and referred to an orthopedic for further evaluation. Orthopedic diagnosed injury as ruptured tendon and scheduled surgery for 8/28/2018.

Employee was climbing a small ladder to access the catwalk of the Deister return conveyor. Upon entering the catwalk the employee grabbed the handrails and transferred all of his weight to his left foot and heard a snap and felt a sharp pain in his left foot. The employee was taken to the local ER to be evaluated and was diagnosed with a fracture of the fifth metatarsal bone.

EE was shoveling under the primary tail pulley conveyor after the plant was shut down and he developed pain in his lower back. This was his normal job. The pain was bad enough that ee had it checked out at the emergency room and they said to rest for 5 days.

EE was prying on a screen cover when slipped and fell dislocating his clavicle. He continued working and later went to the emergency room and they put it back in place. He will be off work for about two weeks.

Because of heavy rain, the crushing crew had to shovel before they could start up. Ee stepped off the concrete pad and his foot slipped in the mud. He tried to catch himself and when he brought his other foot around, he twisted his ankle and caused a fracture in the ankle area.
