AS the EE was opening the gate it jumped off the track causing the gate to come to a sudden and abrupt stop causing sprain/strain injury of the L knee. This was not reported immediately because there was no reportable criteria met until the EE went for an MRI on 4/15/2014.

While fastening a slat while using an impact wrench a foreign object entered the Left eye.
Employee was pulling on a wrench to loosen a hose claim at a water station when he felt something pop in his left hand. This was originally a report only accident. No medical treatment was sought until May 20, 2011.
The employee slipped and fell hitting his face after descending a ladder. This was not reported earlier because it was unknown until recently that the employee was given a prescription medicine to apply topically to the injured area.

Employee was crawling under a dust collector pipe on a silo when he felt his knee pop. This was originally a report only, the employee did not seek medical attention until January 4, 2011.

As he was tightening the bolts into the jacking holes of the bushing with an air impact wrench bushing broke causing the wrench to become vert- ically propelled. causing EE R hand to be crushed in between wrench handle & bottom of mill table.

EMPLOYEE'S REGULAR LOADER WAS PUT INTO SHOP FOR OVERHAUL ON 12-17-94. THE SPARE LOADER HE WAS OPERATING WAS SMALLER AND HAD GEAR SHIFT LEVERS POSITIONED DIFFERENTYL. CLAIMANT IS STATING THAT THE SMALLER LOADER AGGRAVATED HIS BACK PROBLEMS.
