Limeville Quarry: 95 MSHA citations Metal/Non-Metal

AS the EE was opening the gate it jumped off the track causing the gate to come to a sudden and abrupt stop causing sprain/strain injury of the L knee. This was not reported immediately because there was no reportable criteria met until the EE went for an MRI on 4/15/2014.

While fastening a slat while using an impact wrench a foreign object entered the Left eye.

Employee was pulling on a wrench to loosen a hose claim at a water station when he felt something pop in his left hand. This was originally a report only accident. No medical treatment was sought until May 20, 2011.

The employee slipped and fell hitting his face after descending a ladder. This was not reported earlier because it was unknown until recently that the employee was given a prescription medicine to apply topically to the injured area.

Employee was crawling under a dust collector pipe on a silo when he felt his knee pop. This was originally a report only, the employee did not seek medical attention until January 4, 2011.

As the employee was cutting a piece of rubber for a skirtboard, the utility knife that he was using slipped and cut his left hand.

As the employee was loading lime into a tank trailer from #3 Silo, dust blew into his eye.

The EE was in the processof removing a tapered bushing from the seat in the base of #4 Mill. As he was tightening the bolts into the jacking holes of the bushing with an air impact wrench bushing broke causing the wrench to become vert- ically propelled. causing EE R hand to be crushed in between wrench handle & bottom of mill table.

WHILE DISMOUNTING HIS TRUCK, EE STEPPED ONTO A SMALL ROCK AND SUSTAINED A FRACTURE TO HIS RIGHTFOOT.

WHILE WALKING THROUGH THE BAG ROOM, THE EMPLOYEESLIPPED AND FELL ON ACCUMULATED MATERIAL ON THE FLOOR AND SUSTAINED INCREASED NECK PAIN.

EMPLOYEE FELT PAIN IN HIS STOMACH WHILE HE WAS LIFTING 80 LB BAGS OF LIME OFF OF A TELESCOPING CONVEYER AND PLACING THE BAGS ONTO PALLETS.

WHILE GRABBING FOR A HANDHOLD ON A CUSTOMER TRUCK'S SIDEBOARD, SEVERAL SPLINTERS LODGED UNDERNEATH THE EMPLOYEE'S FINGERNAIL.

EE WAS SLIDING A BAG ONTO BAG FILL TUBE WHEN HE JISSED THE BAG VALVE AND HIT HIS FINGER ON THE FILL TUBE, CUTTING HIS FINGER, REQUIRING STITCHES.

EMPLOYEE WAS BAGGING MATERIAL, HE HIT THE BAG NOZZLE WITH THE BACK OF HIS RIGHTHAND CAUSING A LACERATION WHICH REQUIRED STITCHES.

EE WAS LOADING 80 LB BAGS OF MATERIAL ON A TRUCK. EE WAS STOCKING BAGS AND FELT STRAIN IN HIS BACK.

EE HAD LOADED HIS TRUCK & WAS TARPING HIS LOAD. HE SAID HE MISSED A RUNG ON THE LADDER ON TRAILER & LOST HIS BALANCE & FELL.

EMPLOYEE WAS TIGHTENING A BOLT. WHEN THE WRENCH SLIPPED THE EMPLOYEE LOST HIS FOOTING AND FELL HITTING HIS KNEE.

EE WAS LOADING 80# BAGS ON TRUCK WHEN HE FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE HAD A ROCK WEDGED AT CRUSHER BOX. WHEN PRYING WITH A DIGGING IRON THE ROCK KICKED BACK WHICH CAUSED SHOULDER STRAIN AND PARTIAL DISLOCATION.

EMPLOYEE'S REGULAR LOADER WAS PUT INTO SHOP FOR OVERHAUL ON 12-17-94. THE SPARE LOADER HE WAS OPERATING WAS SMALLER AND HAD GEAR SHIFT LEVERS POSITIONED DIFFERENTYL. CLAIMANT IS STATING THAT THE SMALLER LOADER AGGRAVATED HIS BACK PROBLEMS.

EMPLOYEE WAS ASSISTING A CUSTOMER WHO WAS LOADING WET LIME FROM A BIN INTO HIS TRUCK CUSTOMER BEGAN MOVING TRUCK BEFORE BIN GATE WAS CLOSED EMPLOYEE HURRIED TO CLOSE GATE AS HE STARTED UP STEP S OF CATWALK HE CAUGHT HIS TOE OF HIS SHOE ON THE EDGE OF THE FIRST STEP

EMPL WAS ADJUSTING REAR VIEW MIRROR OF R-35 EUCLID. WHILE ATTEMPTING TO ADJUST 1" PIPE WHICH HOLDS MIRROR & IS BOLTED TO FRONT FENDER OF EUC PIPE BROKE LOOSE FROM ITS BASE. EMPL FELL APPROX 6 1/2'.
