EE WAS LOOSENING BOLTS ON BULLDOZER CUTTING BLADE WITH AIR WRENCH. EE MAD CONTACT WITH SPINNING BOLT HEAD WITH RIGHT MIDDLE FINGER CAUSING AN INJURY. EE WAS NOT WEARING ISSUED SAFETY GLOVES. E E WAS TAKEN TO HOSPITAL.

EE SLIPPED WHILE EXITING THE CAB OF TRACTOR #140 & FELL TO THE GROUND. HE CLAIMS THE FALL INJURED HIS BACK. HE WENT TO THE HOSPITAL & HAS BEEN UNDER MEDICAL TREATMENT TO THIS DATE. HE INFORMED SAFTEY DIRECTOR THAT HE IS STILL IN PAIN & WILL RETURN TO THE DOCTOR ON 2/25/02.
