the #2 operator had stopped int he intersection while the feeder was being cleaned and was beside the frontend of s/c. the #1 s/c came from the miner to dump his load of coal and did not see t he #2 s/c in the intersection and hit it. the #2 s/c operator was at the front end of the s/c when it hit.
LIFTING BUCKET OF MINER BITS & TWISTING. MUSCLE STRAIN.
SOMETIME BETWEEN 6 AND 9 PM WE HAD A ROOF FALL AT SPAD #240 IN THE RIGHT CROSS CUT OF THE #5 ENTRY OF THE NO 1 MAINS. I FELL FROM THE #5 ENTRY THROUGH THE CROSS CUT AND INTO THE #6 ENTRY ABOUT 10 FEET. FALL MEASURED 64'L X 18' 20'W X 4'THICK. NO PERSONNEL OR EQUIPMENT INVOLVED. AREA WAS TIMBERED OFF AND FALL WILL NOT BE CLEANED UP.
EMPLOYEE WAS CUTTING CABLE WITH A KNIFE. HE SLIPPED AND CUT HIS KNEE.
EMPLOYEE WAS HAULING COAL WITH A BUGGY, RAN OVER A ROCK AND WAS SLAMMED INTO THE BUGGY'S TOP.
