Employee was helping change a battery on a man trip when battery lead arced and a spark struck employee in face & eye.

While making a pre-shift a fall was found on the #1 belt 30" inby spad 42 measuring 20" wide x 40long x 8"high, after further investgation it was found to a large lip 6' resin bolts was used in complete area.

A fall measuring 20' wide x 4' thick and 30' long was discovered on 2/19/08 at 12:25 pm with a federal mine inspector at spad #382 in the #3 entry. The fall will not be cleaned up build cribs ground fall area and dangered off. The fall is about 2000; from seciton.

At 2:37 pm a roof fall was discovered by the preshift examiner. The fall ranges from 36 to 48" thick, 18' long, 19' wide. The fall will be cleaned up and supported. The location of the fall is at spad 222 in the #4 entry.

FALL WILL BE TIMBERED OFF AND ESCAPEWAY REROUTED. THIS AREA WAS MINED SEPTEMBER 2004 AND BOLTED WITH 4 FT RESIN BOLTS. THIS AREA WAS LAST FIREBOSSED ON 9/22/05.

MINE FOREMAN WAS MAKING FIREBOSS RUN AT APPROXIMATELY 4:30 AM DISCOVERED ROOF FALL IN #3 ENTRY MEASURING 10 FT WIDE X 20 FT LONG X 5 FT THICK. 40 FT INBY SPAD 160. THIS AREA WAS BOLTED WITH 4 FT RESIN BOLTS. FALL WILL BE CLEANED UP AND REBOLTED WITH 6 FT BOLTS AND ANY ADDITIONAL SUPPORT.

Mine Foreman was making fireboss run. At apptox 6"22 am, discovered a roof fall in #3 entry. BRK 41, 1 bk. inby spad 214. Fall measures 20'lx18'wx4'thick. Type of roof bolts used in this area was #5x48". No, person or equipment involved. The wall will be timbered and dangered off.

Employee was walking behind roof bolter when roof bolter operator started reeling cable up on cable reel. The cable was in a mud hole and the employee didn't see the cable being taken up. The cable wrapped around left ankle, throwing employee up into roof of mine and employee landed in mud and water.

Employee was helping put guard on feeder. The other end of guard slipped out of co-worker's hand, shifting all the weight of guard on injured employee. The weight from the guard jammed right hand of injured employee against metal feeder frame.
