A non-employee truck driver slipped on ice while stepping down from the driver's truck and could not stand up. The local EMS was called to transport the driver to a local emergency facility for an evaluation. The driver suffered a fractured femur and had emergency surgery to repair it.

As the EE was cutting through pipe the portable band saw slipped and lacerated his R hand.
While using a cutting torch a piece of slag entered his L eye. This was not reported earlier because the prescription that was given to the EE was not clear on the paperwork received from the Doctor.

As the EE was repairing the roof for the warehouse he strained his R knee. This did not become a recordable injury until he went to the doctors on January 3, 2012. Until that time he was working normally.
EE struck his Left hand with a hammer between the thumb and index finger.
