New Amsterdam Quarry: 133 MSHA citations Metal/Non-Metal

Employee was found in pick up truck unresponsive. Site employees began First Aid/ CPR / AED measures until EMS and First Responders arrived.

Employee was tightening up a fitting on an air starter. When EE was finishing tightening it EE felt a pop in EE's right shoulder. Employee was taken to OMP and has a strained right shoulder with work restrictions.

Employee was coming down a set of steps and on the bottom step rolled an ankle. Was taken to a clinic and given sit stand restrictions.

EE was cutting a piece of center rubber for a screen deck. When making the last cut on the rubber the box cutting knife slipped and struck EE's left leg causing a laceration. EE was taken to the Dr. and evaluated and received 6 stitches on the cut. Accident under investigation.

Employee was putting on a life vest to go onto a barge. As they were putting the life vest on they were bitten by a small insect. Employee was taken to clinic to be evaluated and prescribed steroids for swelling.

EE was closing the window of a Cat 990H. When EE closed the window the right pinky was smashed in the hinge. EE was taken to OMP and the nail was removed and received stitches.

Employee was torch cutting a bolt lose in a feeder liner. A piece of hot slag popped up underneath employee's face shield and safety glasses and a small piece of slag entered the eye. Foreign object removed by eye dr. No other treatment, medicine, no other observation & no other follow up deemed necessary per eye dr.

EE was hauling from the pit to the jaw crushed. When the loader operator loaded EE's truck a larger rock rolled into the back of truck hitting the side of the bed and jarring the truck, thus causing EE to have some lower back pain. EE was taken to be evaluated and is being treated for a pinched nerve. Dr. prescribed a steroid and physical therapy.

EE was trying to remove a suspended deflector plate when the left index finger came between the lift chain and the plate. This resulted in a broken/smashed index finger. EE was taken to the hospital and treated for a broken/smashed finger and received stitches. EE was also prescribed antibiotics and pain medicine.

EE was helping change screen at the plant. Another employee helping moved the screen cloth and set it on EE's left hand. This resulted in EE's Left hand Ring finger being smashed. First aid was administered at the quarry. The employee received medical attention getting stitches on ring finger on the left hand.

Preparing to repair a dump belt splice on a 18 ft x 72 inch conveyor belt. The belt was locked, tagged and verified then the drive belt guard was removed. Injured was using both hands to pull & push on the v-belt drive belts. EE caught index finger between the v-belt and the pulley. Fracture of the distal phalanx left index finger.

EE was cleaning berm from in front of spoil/stripping ledge prior to break time. Material from the ledge slid down the wall onto the bucket down the boom and through the windshield of the cab. Victim exited loader and sought first aid. EE was sent to medical care, and diagnosed with lacerations and contusions. EE was perpendicular to the face and material was rock and mud.

Employee suspects to have re-aggravated EE's back while operating the loader all day the prior to seeking medical treatment. EE cannot identify what or when EE's injury occurred.

Lifting a light fixture from shoulder height to ground felt a pop in lower back. Fixture weighed 30 - 40 pounds.

EE was filling and lifting 5" bags for powder truck. Felt twinge in back.

Upon leaving the quarry to travel to other mine he felt a burning feeling in his eye. He said the wind was wild because of weather front coming in. He called safety and said something is in his eye. He went to eye doctor - rust speck was removed off the eye. Eye doctor administered Rx eye drops as required by Indiana law. EE wears safety eye wear.

Employee was climbing down the ladder of the CAT loader. When he reached the ground, employee stepped awkwardly on the dirt on the ground, rolling his left ankle.

EE was using a 5 foot pry bar to line up hole on crane boom to install a pin and he felt something in his foot making a loud pop.

EE was exiting the forklift correctly. When stepping down from lift onto the concrete his ankle rolled causing a sprain to the ankle. He was given restricted duty work.

EE had removed some metal from the detector on the conveyor, and a softball size rock bounced off of the conveyor that had just recently restarted, and somehow struck him on his neck and shoulder. He was taken to medical care facility for an examination. He was given one day of restrictions until tests came back. He is already back full duty.

Using a hydraulic jack mounted in a steel frame to press out a king pin in a 35 ton truck. The steel frame and jack fell and struck him on the right leg and ankle resulting in a bruised and sprained right ankle.

Welding on a piece of angle iron. Removed glove to get a drink of water then picked up hot angle iron before putting glove back on resulting in a burn to his right hand.

Walking to electrical building to get supplies, slipped on ice and fell on left elbow causing a bruised elbow.

Walking down a flight of stairs felt sharp pain in knee resulting in a strained knee.

Driving across the yard to dump a load of stone, he struck another truck on the side causing a strain to his neck.

Driving across yard to the plant to get loaded, was struck on side of his truck by another truck causing a strain to his neck.

Using a front end loader, loading rock. The loader bounced and dirt came out of the AC vent and blew into his eye, causing a scratched eye.

While replacing ski ramp planks he stepped through a hole in the ski ramp and fell. He was carrying a plank which dropped on his little finger causing a laceration requiring sutures.

Stepping off crane barge onto ski ramp. Stepped on a line causing him to turn his ankle resulting in a sprained ankle.

Walking across concrete floor, slipped on some packing material and strained back. Was not reported until 4/20/07.

Victim was inside a truck, dumping spoil, when an explosive shot was fired in the quarry pit. A piece of fly rock traveled over the highwall and about 1200 ' striking the EE, causing a laceration requiring sutures.

Performing maintenance on a day when the temperature was in the high 90's. Got overheated resulting in heat exhaustion.

EE fell from landout platform striking his head causing a laceration requiring sutures.

Lifting an oxygen cylinder to put on the back of the welding truck and pulled a muscle on his left side resulting in a strained muscle in his groin.

Removing punch plates from a screen, one of the punch plates slipped and mashed the victim's thumb between the plate and frame causing a fracture.

Cleaning parts of a new cone liner, the liner rolled onto his foot causing a broken bone on the top of his foot behind the steel toe.

Installing an axle cap on a loader wheel. The cap slipped causing a laceration to his right hand, requiring stitches.

EMPLOYEE CUT THE TOP OUT OF A STEEL BARREL AND CAUGHT HIS FINGER ON A SHARP EDGE, RESULTING IN A LACERATION TO HIS FINGER THAT REQUIRED SUTURES.

Picking up a bottle off the floor. When he raised up he hit his ear on drawer of his truck tool box causing a laceration requiring sutures to his right ear.

USING A LOADER TO CLEAN A BARGE FLOOR. THE LOADER BUCKET HIT A PLACE IN THE BARGE FLOOR THAT CAUSED THE LOADER TO STOP SUDDENLY RESULTING IN A STRAIN TO HIS BACK.

A ROCK HIT AND BROKE THE HANDRAIL ON AN R-35 HAUL TRUCK. THE DRIVER WENT TO THE SHOP AND HAD THE HANDRAIL WELDED AND REPAIRED. THE DRIVER WAS GETTING BACK INTO THE TRUCK AND TOUCHED THE HOT HANDRAIL CAUSING A BURN TO HIS LEFT HAND.

Cleaning a barge floor using a loader to push coal to one end. The loader bucket hit a bad spot in the floor causing the loader to stop suddenly causing his left hand to hit the steering wheel resulting in a sprained hand.

Installing bolts in a skiry board on a conveyor wind blew dust into eye behind safety glasses causing a scratch to his eye.

Employee was bringing steel up out of the hole. He saw the dust and started running. A rock fell from ledge and hit employee's ankle.

Victim was cutting a piece of metal. A hot piece fell and burned through his coat sleeve and down into his glove resulting in a burn to his right forearm.

CLIMBING OVER A HANDRAIL TO GET ON TOP OF A CONVEYOR BELT AND STRUCK RIGHT KNEE ON THE HANDRAIL CAUSING A BRUISED RIGHT KNEE.

EMPLOYEE WAS GETTING OUT OF HIS TRUCK TO STEP ONTO A WALKWAY. HE SLIPPED & FELL BETWEEN THE WALKWAY & THE TRUCK, RESULTING IN A BRUISE TO HIS LOWER BACK AND ELBOW.

EMPLOYEE WAS CUTTING BRUSH & BENT OVER TO PICK UP SOME LIMBS & TWISTED HIS BACK CAUSING A BACK STRAIN.

REMOVING ROCK FROM A CATWALK, SLIPPED ON A ROCK & FELL ON KNEE & SKINNED LEG RESULTING IN A BRUISED KNEE.

EE WAS PUTTING A CABLE ON A WINCH USING A HAMMER TO GET THE CABLE IN POSITION. HE MISSED THE CABLE WITH THE HAMMER AND STRUCK HIS HAND, CAUSING A BRUISED HAND.

VICTIM WAS CARRYING A WATER PUMP. THE GASOLINE CAP ON THE PUMP WAS LOOSE AND SPILLED GASOLINE ON THE EMPLOYEES CLOTHES. HE SET THE PUMP DOWN QUICKLY AND TWISTED THIS ANKLE CAUSING A SPRAINED ANKLE.

CLIMBING INTO THE BACK OF HIS TRUCK PULLING HIMSELF UP WHEN HE PULLED HIS SHOULDER OUT OF PLACE CAUSING A STRAIN TO HIS LEFT SHOULDER.

CLIMBING THE LADDER ON A LOADER, SLIPPED ON A STEP AND TWISTED HIS KNEE CAUSING A KNIFE STRAIN.

USING A PRY BAR TO PRY ON PART. THE PRY BAR SLIPPED OFF THE SURFACE THROWING THE VICTIM OFF BALANCE. HE FELL BACKWARDS STRIKING HIS HEAD ON A HARD SURFACE CAUSING A LACERATION REQUIRING SUTURES.

EE WAS LIFTING A DRILL SUBSHAFT, WHEN SHE PLACED IT ON THE FLOOR IT DROPPED AND FELL ON HER LT. INDEX FINGER CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS HELPING TO MOVE A SKI RAMP. HE TURNED TO WALK AWAY AND TWISTED HIS KNEE CAUSING A STRAIN TO HIS LEFT KNEE.

EE WAS CHECKING A BARGE AS IT WAS BEING LOADED. A WIRE CABLE BROKE & HE TRIED TO JUMP OUT OF THE WAY. THE CABLE STRUCK HIS ARM AND HE FELL CAUSING A FRACTURE TO HIS RIGHT ARM.

EXITING A PIECE OF EQUIPMENT GOING DOWN THE STEPS. WHEN HE REACHED THE BOTTOM HE TWISTED HIS ANKLE CAUSING A SPRAIN TO THE RT. ANKLE.

PULLING A WIRE CABLE AS HE WAS WALKING & STEPPED ON A ROCK CAUSING HIM TO FALL ON HIS RIGHT HAND RESULTING IN A STRAIN TO HIS RIGHT WRIST.

CLIMBING OUT OF A BARGE HIS FOOT SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND STRUCK HIS ELBOW AGAINST THE SIDE OF THE BARGE CAUSING A BRUISED ELBOW.

EE WAS PUNCHING HOLES IN A CONVEYOR BELT, USING A HOLE PUNCH AND HAMMER. HE MISSED HITTING THE PUNCH AND STRUCK, HIS THUMB WITH THE HAMMER CAUSING A LACERATION REQUIRING SUTURES.

PULLING A WIRE CABLE TO HELP SWITCH A BARGE AND PULLED A MUSCLE IN HIS BACK CAUSING A BACK STRAIN.

VICTIM WAS CLEANING UNDER A CONVEYOR AND STARTED TO WALK AWAY FROM THE CONVEYOR WHEN HE STEPPED ON A LARGE ROCK CAUSING HIM TO SPRAIN HIS ANKLE.

VICTIM WAS HELPING MOVE A BARGE WHEN A WIRE CABLE BROKE CAUSING HIM TO FALL. WHEN HE FELL HE STRUCK HIS NOSE ON A PULLEY CAUSING A LACERATION TO HIS NOSE.

VICTIM SLIPPED ON THE CATWALK OF AN 435 HAUL TRUCK CAUSING HIS LEG TO DROP BETWEEN THE HANDRAIL AND THE DOOR. HIS LEFT KNEE HIT THE CATWALK CAUSING A BRUISED LEFT KNEE. INCIDENT WAS REPORTED O N 4-17-01 TO HIS SUPERVISOR.

VICTIM & WITNESS WERE PULLING A WIRE CABLE TO HELP SWITCH A BARGE. HE PULLED TOO HARD & STRAINEDA MUSCLE IN HIS BACK. HE DID NOT HAVE PAIN UNTILTHE NEXT DAY WHEN HE WENT TO THE DOCTOR. HE REPO RTED TO HIS SUPERVISOR ON 4/6/01.

VICTIM PICKED UP A ROCK AND WAS WALKING TO THE JAW CRUSHER WHEN HE TRIPPED ON ANOTHER ROCK ON THE GROUND CAUSING A STRAIN TO HIS LOWER BACK.

STEPPING FROM A STEPS TO A BARGE, STEPPED ON SOME LOOSE ROCK AND FELL CAUSING A STRAINED KNEE AND LOWER BACK.

VICTIM WAS SITTING IN A LOADER ABOUT 1270' FROM A SHOT OF DYNAMITE BEING FIRED. A PIECE OF FLY ROCK FROM THE SHOT PASSED THROUGH THE WINDSHIELD OF THE LOADER STRIKING THE VICTIM ON THE SIDE OF HIS FACE CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS CUTTING A PIECE OF STEEL H-BEAM; THE BEAM SHIFTED AND ROLLED ONTO HIS ANKLE, CAUSING A BRUISED AND SWOLLEN ANKLE.

VICTIM WAS CLEANING A RETURN ROLLER. HE STRUCK THE GUARD WITH A HAMMER CAUSING THE GUARD TO FALLAND STRIKE HIS FINGER CAUSING A LACERATION TO HIS RIGHT LITTLE FINGER.

VICTIM WAS MOVING A FILING CABNET AND PINCHED HIS FINGER BETWEEN THE FILING CABINET AND A SHELF CAUSING A BRUISED FINGER.

VICTIM WAS USING A WRENCH TO REMOVE A STARTER. THE WRENCH SLIPPED OFF THE BOLT AN STRUCK THE VICTIM ON THE RIGHT ELBOW CAUSING A CONTUSION TO THE ELBOW.

VICTIM WAS UNBOLTING AN ELECTRICAL WITCH WEHN THE SWITCH FELL ON HIS LEFT HAND AND SMASED HIS LEFT RING FINGER.

VICTIM WAS DRIVING AN R85 HAUL TRUCK AND SAID IT CAUSED HIM TO HAVE A BACK STRAIN.

VICTIM WAS PICKING UP A WATER PUMP AND PULLED A MSUCLE IN HIS BACK CAUSING A LOWER BACK STRAIN.

VICTIM WAS USING A WRENCH TRYING TO OPEN A DRAIN PLUG ON A CRAN WHEN THE WRENCH SLIPPED OFF THE PLUG CAUSING HIM TO HIT HIS FINGER ON THE PLUG RESULTING IN A LACERATION TO HIS RIGHT MIDDLE FIN GER REQUIRING SUTURES.

VICTIM WAS STEPPING OFF OF AN 8 INCH HIGH PLATFORM WHEN SHE TWISTED HER LEFT ANKLE CAUSING A SPRAINED ANKLE.

VICTIM WAS REACHING UP TO GREASE A GREASE FITTING AND PULLED A MUSCLE CAUSING A BACK STRAIN.

VICTIM WAS STANDING ON A CTAWALK PREPARING TO START WORK WHEN THE WIND BLEW A PIECE OF DUST INTO HIS EYE CAUSING A SCRATCH TO HIS EYE.

VICTIM WAS GETTING OFF A LOADER WHEN HIS RIGHT LEG GOT CAUGHT AND HE FELL CAUSING PULLED LIGAMENTS IN HIS RIGHT LEG.

VICTIM WAS LOADING EQUIPMENT ONTO A TRUCK AND PULLED A MUSCLE IN HIS BACK CAUSING A STRAINED BACK. HE DID NOT HAVE PROBLEMS WITH IT UNTIL 3-23-99. HE THEN WENT TO A DR AND RECEIVED MEDICATION FOR HIS BACK.
