EE WAS OPERATIN1 A BOBCAT LOADER. THERE WAS A ROCK UNDER THE PEDALS. THE BUCKET WAS ON THE GROUND ON ITS NOSE & THE ENGINE WAS RUNNING. EE WENT TO THE FRONT OF THE MACHINE TO REMOVE THE ROCK EE REMOVED THE3ROCK & MACHINE TRIED TO LOWER THE BUCKET WHICH WAS ALREADY ON THE GROUND. THIS CAUSED THE FRONT OF T HE MACHINE TO LIFT UP. Maintenance worker used a torch to cut a bolt off of a conveyor idler. They hit the idler with a hammer and the piece of bolt they had cut off flew up and hit them in the right eye. (safety glasses were slid down on the tip of nose and did not protect from injury.) The result of the incident was a hematoma on the eye and a scratched cornea.

The employee was reaching to turn off the water hose that the employee had been using to wash out around a conveyor belt. Employee lost balance when employee reached to turn off the hose. Employee fell over against a newly poured concrete wall. The concrete form wires had not been properly removed and there was an exposed wire left sticking out that cut employee's arm.

EE was loading a flat deck company barge (ORS-722) at the upper dock. The barge was about 75% loaded when EE stepped up onto the barge that was approx. 12" higher than the dock level with right foot. Using hands EE pulled self up and right foot slipped in a small amount of sand causing right knee to hit the barge resulting in swelling and a sprain knee.

Setting in a 16 foot "I" beam with come-a-longs. Beam became hung on scalper. EE pulled the beam loose with left hand. The beam swung out and pinched left index finger between the beam and a handrail. EE was wearing gloves at the time. The impact of the beam lacerated the end of the finger, resulting in stitches.
