Mulzer Crushed Stone Inc · Fall to the walkway or working surface

EE was stepping over an anchor winch wire on #15 Dredge, and then immediately turned 180 degrees to go back and tripped over winch wire EE just stepped over. EE placed both hands down to catch fall. EE's body weight with EE's fully straightened arms caused a compression injury causing a fracture to the elbow joint of EE's forearm upon impact.

EE and the crew went to bank and stepped off of the boat and felt a slight discomfort in his foot. He stated that it was nothing, but by Wednesday May 2nd his foot was stiff and hurt. He requested medical treatment and was taken to the clinic. He reported it on May 2, 2012. EE is our weekend watchmen on the dredge.

EE was exposed to paint fumes from a neighboring bay that irritated his lungs. He requested doctor visit, and he was given a prescription inhaler for lung irratation and told to visit his family physcian for an upper respiratory infection which was not ruled work related per Comp Center. The Dr. did indicate he had some irratation that was caused by fumes - requiring meds.

EE was working on repairing a crane bucket and stepped onto the raw steel of the bucket when his foot slipped causing him to slide resulting in a strain/sprain of his right knee.

EE was moving a fan in the dry dock area when dust/rust blew from surrounding fans around his PPE and landed on his eye. He naturally rubbed it which resulted in a small scratch on his eye. It was removed and he was prescribed an prescription antibiotic. Released immediately to full duty no restrictions.

EE was welding on a part in the machine shop when a piece of slag enter his eye. Employee was wearing proper PPE, and is not sure how the piece reached his eye. He was unaware that a piece embedded on his eye until bedtime that evening. He had the piece removed and reported it on 7/28/11. Prescription eye drops were prescribed to prevent infection.

EE was scraping the deck plates on Joyce A (switch boat),while docked at Rockport, and he struck his hand with the scraper causing a small laceration requiring 2 stitches.

EE was retrieving a piece of expanded metal out of rack, and caught his right calf on a piece of metal. It cut through his jeans, and lacerated his leg requiring a few stitches to close the wound. He was wearing all required PPE.

EE was chipping hot slag on river barge, and a piece of hot slag bounced and entered his full face welding helmet and safety glasses and rested on his eye. He flushed it quickly, and the foreign object was washed out. EE was sent to eye doctor to have it checked, and was given antibiotic eye drops to take for three days. It was diagnosed as a superficial burn on the eye.
