Rockport #15 Dredge: 42 MSHA citations Metal/Non-Metal

EE and the crew went to bank and stepped off of the boat and felt a slight discomfort in his foot. He stated that it was nothing, but by Wednesday May 2nd his foot was stiff and hurt. He requested medical treatment and was taken to the clinic. He reported it on May 2, 2012. EE is our weekend watchmen on the dredge.

EE was exposed to paint fumes from a neighboring bay that irritated his lungs. He requested doctor visit, and he was given a prescription inhaler for lung irratation and told to visit his family physcian for an upper respiratory infection which was not ruled work related per Comp Center. The Dr. did indicate he had some irratation that was caused by fumes - requiring meds.

EE was working on repairing a crane bucket and stepped onto the raw steel of the bucket when his foot slipped causing him to slide resulting in a strain/sprain of his right knee.

EE was moving a fan in the dry dock area when dust/rust blew from surrounding fans around his PPE and landed on his eye. He naturally rubbed it which resulted in a small scratch on his eye. It was removed and he was prescribed an prescription antibiotic. Released immediately to full duty no restrictions.

EE was welding on a part in the machine shop when a piece of slag enter his eye. Employee was wearing proper PPE, and is not sure how the piece reached his eye. He was unaware that a piece embedded on his eye until bedtime that evening. He had the piece removed and reported it on 7/28/11. Prescription eye drops were prescribed to prevent infection.

EE was scraping the deck plates on Joyce A (switch boat),while docked at Rockport, and he struck his hand with the scraper causing a small laceration requiring 2 stitches.

EE was retrieving a piece of expanded metal out of rack, and caught his right calf on a piece of metal. It cut through his jeans, and lacerated his leg requiring a few stitches to close the wound. He was wearing all required PPE.

EE was chipping hot slag on river barge, and a piece of hot slag bounced and entered his full face welding helmet and safety glasses and rested on his eye. He flushed it quickly, and the foreign object was washed out. EE was sent to eye doctor to have it checked, and was given antibiotic eye drops to take for three days. It was diagnosed as a superficial burn on the eye.

hooking up a lift chain to remove a loader bucket. The loader bucket had a burr on it that caused a cut to his right little finger, requiring sutures.

Squatting down to weld a hatch cover on a barge his pants leg caught on fire causing a second degree burn to his left leg.

Climbing a ladder. The ladder slipped and he fell to the floor, resulting in a broken leg.

Using a screwdriver to tighten a hose clamp on a hose the screwdriver slipped off the hose clamp and struck him on the right hand causing a laceration requiring sutures.

Removing a compressed gas cylinder from a rack, the cylinder fell against another cylinder pinching his finger between them causing a laceration requiring sutures.

Stepping from dock to barge a cable tightened and struck him in his knee causing a bruised knee.

PINCHED RING FINGER BETWEEN TWO BARRELS CAUSING A CONTUSION TO HIS RIGHT RING FINGER.

CLEANING IN A WAREHOUSE MOVING BOXES AND OTHER ITEMS AND WAS APPARENTLY BITEN BY A SPIDER CAUSING HIM TO FEEL NAUSEOUS.

EE WAS DESCENDING A STEP, FEET SLIPPED OUT FROM UNDER EE AND FELL LANDING ON BACK AND ELBOW RESULTING IN A BACK STRAIN.

EMPLOYEE WAS USING A BRAKE PRESS TO BEND A PIECE OF PLATE STEEL. THE STEEL WAS NOT POSITIONED STRAIGHT WITH THE PRESS. WHEN HE ATTEMPTED TO BEND THE STEEL, IT FLIPPED UP STRIKING THE EMPLOYEE ON THE CHIN CAUSING A CHIPPED TOOTH.

EE WAS USING A BAND SAW TO CUT A PIECE OF CHANNEL. WHEN HE PLACED THE CHANNEL IN THE VISE TO SECURE IT FOR CUTTING HE GOT HIS FINGER CAUGHT BETWEEN THE VISE AND CHANNEL, CAUSING A LACERATION REQUIRING SUTURES AND PARTIAL AMPUTATION TO FINGER - NO BONE INVOLVEMENT.

TWO EMPLOYEES WERE MAKING ADJUSTMENTS TO A CONVEYOR. A THIRD EE ACTIVATED THE CONVEYOR THEY WERE WORKING OFF OF, CAUSING ONE EE TO FALL, LANDING ON HIS BUTTOCKS AND HANDS. THIS RESULTED IN TWO BROKEN WRISTS. EMPLOYEES WERE NOT FOLLOWING WORK POLICY AND PROCEDURE.

VICTIM WAS USING A HAND GRINDER REMOVING BURRS FROM A PIECE OF STEEL PLATE. THE STEEL PIECE STARTED TO MOVE. HE TRIED TO STOP IT WITH HIS HAND & CAUGHT HIS FINGER ON A SHARP EDGE CAUSING A LA CERATION TO HIS FINGER REQUIRING SUTURES.

SLIDING AN OXYGEN BOTTLE INTO A CAGE. GOT HIS RIGHT WRIST CAUGHT BETWEEN THE BOTTLE & THE CAGE CAUSING A BRUISED WRIST.

VICTIM WAS LIFTING AN EMPTY FUEL TANK AND STRAINED HIS BACK.

CUTTING A PIECE OF STEEL. THE STEEL SHIFTED BEFORE HE GOT IT CUT AND PINCHED HIS FINGER BETWEEN THE PIECE OF STEEL AND A STEEL ANGLE CAUSING A CUT TO HIS FINGER REQUIRING SUTURES.

USING A BAND SAW TO CUT A PIECE OF STEEL. WHILE SECURING THE PIECE OF STEEL IN THE VISE GOT HIS THUMB CAUGHT IN THE VISE BETWEEN THE PIECE OF STEEL AND THE VISE CAUSING A LACERATION REQUIRING SUTURES.

TRYING TO ROTATE A STEEL RUDDER. WHEN THE RUDDER DID ROTATE HIS THUMB WAS BETWEEN THE RUDDER AND THE HOUSING CAUSING HIS RIGHT THUMB TO BE MASHED CAUSING A LACERATION REQUIRING SUTURES.

VICTIM BENT OVER TO SLIDE A WATER PUMP ACROSS THE FLOOR AND STRAINED HIS BACK.

EMPLOYEE WAS CUTTING A PIECE OF 6' STEEL BEAM. APIECE OF STEEL FELL ON HIS FOOT ABOVE THE STEEL TOE AND FELL ON HIS FOOT CAUSING A FRACTURE TO THE BIG TOE.

VICTIM WAS DRIVING OUT A STEEL BUSHING WITH A HAMMER AND STRUCK HIS THUMB WITH THE HAMMER CAUSING A LACERATION REQUIRING SUTURES.

REMOVING STEEL PANELS FROM A CRANE. A PANEL FELL ON HIS LEFT HAND CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS CUTTING A PIECE OF STEEL WHEN THE PIECE OF STEEL SCRAPED HIS FINGER CAUSING A LACERATION TO HIS FINGER REQUIRING SUTURES.

REPLACING A CYCLINDER. THE CYCLINDER HAD A BURR THAT VICTIM DID NOT SEE AND CUT HIS ARM ON THE CYCLINER REQUIRING SUTURES.

REMOVING A PIN FROM A TRANSMISSION USING HEAT AND PRESSURE. THE PIN SPRUNG LOOSE AND STRUCK THE VICTIM ON HIS RIGHT HAND CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS SETTING UP A SCAFFOLD AND PUTTING BOARDS ON THE SCAFFOLD WHEN HE CRUSHED HIS HAND CAUSING A BROKEN THUMB.

VICTIM WAS BENDING OVER TO WORK ON A CHAIN AND SPRAINED HIS BACK.

VICTIM WAS WORKING ON AN OVERHEAD CRANE AND PUNCTURED HIS HAND ON A PIECE OF STEEL CAUSING A LACERATION.

USING A STEAMER SPRAYING OFF A TRCUK COUPLING CAME APART & HOT WATER WAS SPRAYED ON HIS STOMACH AREA CAUSING BURNS I DID NOT KNOW OF THIS ACCIDENT UNTIL TODAY 09-20-91

CUTTING PIECE OF STEEL WHEN CUT WAS COMPLETED PIECE OF STEEL FELL ON TOP OF FOOT CAUSING BONE FX.

EMP PICKED UP A3PIECE OF STEEL THAT FELL OFF A FORKLIFT AND STRAINED HIS BACK

EMP WAS BENDING3OVER CLEANING A FLOOR AND STRAINED HIS BACK

EMPLOYEE WAS PI7KING UP A PIECE OF STEEL AND STRAINED HIS BACK.

EMPLOYEE HAD UN2OOKED A CRANE CABLE FROM SOME PIPE AS THE CABLE WAS RAISED A CABLE HOOK CAUGHT SOME STEEL PILING WHICH FELL AND SCRAPED THE BACK OF BOTH LEGS RESULTING IN LEG MUSCLE BRUISES.

EMPLOYEE WAS MO5ING A STEEL ANGLE BY HAND AND STRAINED HIS BACK.

A WATER PUMP WAS SITTING ON THE SIDE RAIL OF A BARGE PUMPING WATER OUT OF THE CARGO BOX. THEPUMP TIPPED AND THE VICTIM TRIED TO CATCH IT AND CAUGHT HIS THUMB IN THE FLY WHEEL.
