EE WAS HAVING TROUBLE WITH HIS SHUTTLE CAR REEL NOT SPOOLING PROPERLY AND ASKED THE SECTION ELKECTRICIAN TO ASSIST HIM WITH HIS CABLE. THE ELECTRICIAN WAS CLEANING GOB FROM AROUND THE REEL WHI LE HE WAS TO TAKE THE CABLE ANCHOR APART. WHEN THE ELECTRICIAN LOOKED AROUND THE EE WAS LYING ON MINE FLOOR UNRESPONSIVE.
A ROOF FALL OCCURED ON THE 001-0 SECTION, IN THE #4 ENTRY, INBY SPAD #34989. THE FALL MEASURED 8' WIDE BY 20'TO 26' LONG AND 4'HIGH.
EE GOT OFF SHUTTLE CAR AND TOLD FOREMAN TO RESET A BREAKER. ON HIS WAY BACK A ROCK FELL STRIKING HIM.
