EMPLOYEE WAS ENGAGED IN INSTALLING PERMANENT ROOF SUPPORT. HE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT WHEN THE BOLT BECAME FOULED IN THE ROOF. HE ATTEMPTED TO APPLY UPWARD PRESSURE WITH H IS DRILL POT BUT, ACCIDENTALLY ENGAGED THE ROTATION FUNCTION INSTEAD. THE ROTATING MOTION OF THE ROOF BOLT PUSHED HIS HAND BACK TOWARD HIS FOREARM.
