Employee was lifting an auger bit on the rack and dropped it. It pinched/crushed the tip of left ring finger.

Employee was washing the pad under and around the plant. EE stood up and hit head on the frame of the plant. The angle iron support caused a laceration to the right eye/forehead.

The plant was shut down at 3:30 pm because of a rock in the impeller on the dredge. After this task, 2 miners were at the controls in the plant electrical trailer to start the plant back up at 4:00 pm. It was within a few seconds of pushing the slurry pump start button, that the 2 miners heard what sounded like an arcing sound & then a loud bang
