Lyman Richey Corp dba L R S&G Co · Accident type, without injuries

Operator of the Komatsu WA500 Front End Loader parked the FEL parallel to the Loadout face of the stockpile. The Operator dismounted the FEL to go to the restroom inside the pivot area of the FEL. As the operator unfastened their bib overalls, the stockpile sloughed hitting the miner in the back of the legs, knocking them down to their knees, pinning the against the FEL.

Injury was a first aid from 02/17/2020, it became record-able on 03/06/2020. Employee slipped and fell on the same level in a twin sand dewatering screw. Employee has a non displaced fracture of the lower right arm.

Employee was washing out the log washer and felt a pain in calf causing employee to slip. Hyperextended right knee. Did not report injury until 5/22/17 and did not report it as work related until 5/25/17.
