Employee was walking from Primary tower and slipped on a rock and twisted knee.

Employee was installing a new hose under crusher CR-3. The hose became stuck and the employee pulled on the hose to free it. The hose broke loose and the employee lost his footing and fell back against the side of the crusher. Employee suffered back strain.

4 Employees were tightening nuts on crusher using 20 lb sledge. A metal piece splintered off and struck employee in the lower R abdomen.

Employee was welding wear strips to floor of primary feeder, took shield off and began to chip slag. A piece of slag got behind his safety glasses and into his left eye. It irritated his left eye.

Employee was checking spray bar, slipped on an incline and banged right leg on a bolt that was sticking out which caused a contusion on his right lower leg.
He was using a come-along to put steel plate in chute and developed pain in his right elbow.

Was removing a nitrogen cylinder for the ElJay crusher. Oil under pressure burst from the cylinder into his eye. Eye protection was being worn at the time.
