While EE was setting up for a repair to the jaw plant, EE had to pull welding leads. EE went to secure them on the side of the plant but missed; the weight of the leads pulled EE off balance. At that point, EE knew was going to fall off the step ladder so EE turned to step to the ground. EE's left foot landed on a rock that rolled EE's ankle causing the injuries.

CRUSHING PLANT OPERATOR REMOVING PORTABLE CONTROL PANEL DURING PLANT DISASSEMBLY LOWERED IT FROM UPPER LEVEL TO SECOND LEVEL. OPERATOR WENT TO GROUND LEVEL TO REMOVE BOX FROM PLANT AND THE BOX FELL HITTING HIM IN THE HEAD REQUIRING 13 STICHES IN HIS HEAD.

HE WAS ATTEMPTING TO SET A MOTOR ON THE SCREEN SECTION OF OUR CLOSED CIRCUIT PLANT INTO ITS OPERATING POSITION WHEN THE CHAIN CAME OFF CAUSING THE MOTOR TO SWING DOWN AND HIT HIS 3RD AND 4TH F INGERS ON HIS RIGHT HAND AGAINST THE MOTOR SUPPORT. BONES WERE BROKEN IN BOTH FINGERS.
