Revelation Energy, LLC · Struck against a moving object

Employee states that he was doing reclamation work and while coming down over a slope he got on a rock and lost control and began sliding. He states that he let the blade down to slow him and it almost stopped the dozer. When the dozer almost stopped it slung him forward.

Employee started up from th epit on an access road in order to construct a drill bench. He was operating dozer approx 15 ft up the access road the dozer got over too far left over the berm and started to slide towards the pit. Immediately material was being placed under the dozer where it had partially went over the edge. He tried to turn his dozer toward the pit.

Building road dozer contact with tree and hanging dead limb fell and hit and broke side glass and hit EE in the left eye. Flushed on site and transported to ER where he was checked and released with no damage to L. eye, EE was wearing his glasses.

Supervisor was taking water at lunch to Pit 3. When he pulled up EE was standing by loader and said he fell off bottom step of truck, EE said he must of bent step early in the day. Supervisor took EE to parking lot where EE's wife drove him to the Dr.

EE was parking front end loader at the end of the shift. EE caught his foot on bottom step and fell, hitting his head. Lacerated the right side of his head.
