EE (Contractor: Mobile Contract Services LLC representing Navman Wireless) was climbing down from cab of a Cat Grader while inspecting for install of telematics GPS equipment, and on the ground, he stepped back and tripped on grader blade that was parked at an angle, falling backwards. While attempting to catch his fall, he broke his left wrist.

Employee was working on the removal of a conveyor bearing. Employees thumb got caught between a come along and the bearing. This incident was reported as a First Aid, however, after a few weeks employee felt some discomfort on his thumb, and on 12/03/2014 he was taken to a doctor, who placed him on restricted duty until results of x-rays were available.

EE WAS REPLACING DRAGLINE FRICTION BAND BY HIMSELF. HE LOST CONTROL OF BAND AND IT FELL ON FINGERS OF LEFT HAND. SHOULD HAVE HAD ASSISTANCE OF ANOTHER EMPLOYEE OR USED OVERHEAD CRANE TO CON TROL PLACEMENT OF FRICTION BAND.
EE WAS REPLACING A DRAG FRICITON BAND. HE PULLED UP ON DEAD END SWIVEL TO LINE UP HOLES IN THE SWIVEL WITH EYE OF THE DEAD END OF THE BAND. WHILE DOING SO HE PLACED HIS RT MIDDLE FINGER THRU T HE HOLE IN THE SWIVEL AND PARTIALLY INTO THE EYE OF THE FRICTION BAND. SUDDENLY THE BAND, WHICH WAS UNSECURED, SPRANG OUT OF SWIVEL, CUTTING OFF 3/8" OF MIDDLE FINGER.

EMPLOYEE WAS USING A 3-FOOT X 2-9INCH DIAM PIPE AS AN EXTENSION ON A WRENCH TO TIGHTEN HOIST BRAKE ADJUSTMENT BOLT ON TOP OF MANITOWOC 4600 DRAGLINE. AS HE PUSHED EXTENSION PIPE, IT SLIPPED OFF AND SMASHED HIS FINGER AGINST THE STEEL STRUCTURE ON TOP OF DRAGLINE.
